摘 要:目的探讨缺血性脑卒中大鼠发生肝损害(HSI)的机制及与肠黏膜屏障(IMB)损伤和肠源性内毒素血症(GET)的关系。方法 48只雄性健康Wistar大鼠随机分为2组,缺血性脑卒中模型组和假手术组,模型组根据实验终止时间分为5个亚组,每组动物8只。采用Longa线栓法建立大鼠大脑中动脉闭塞(MCAO)模型。分别于缺血后3、6、12、24和48处死动物,测定各组大鼠血清谷丙转氨酶(ALT)、天门冬氨酸转氨酶(AST)和血浆内毒素(LPS)水平,光镜及电镜下观察肝组织及小肠黏膜超微结构的改变。结果脑卒中各亚组血清肝酶水平均明显高于对照组;与对照组比较差异有统计学意义(P<0.05);脑卒中后各亚组血浆LPS水平显著高于对照组差异有统计学意义(P<0.05),且LPS水平与血清肝酶呈显著正相关;TBI各组光镜和电镜下肝组织和肠黏膜上皮细胞均有不同程度受损。结论采用改良的Longa线栓方法,可以成功地建立缺血性脑卒中后肝、肠应激性损伤的动物模型。脑卒中后肝损伤可能与肠源性内毒素血症(GET)有关。Objective To discuss the mechanism of liver injury followed ischemic stroke of rats and the relation between intestinal mucosal barrier and gutdirived endotoxemia. Methods 48 healthy adult male Wistar rats were divided into two groups randomly: bogus operation group and pMCAO group. According to the final time of experiment, pMCAO group was further divided into five groups, and each group included 8 rats. pMCAO group used improved Longa suture- occuluded method to establish focal cerebral ischemia mode. To kill the animals at 3 hours, 6 hours, 12 hours, 24 hours and 48 hours after ischemia. Testing the level of Serum ALT, AST, and plasma endotoxin(LPS), and observing histological and ultrastructural changes of liver and small intestinal mucosa by optical microscope and transmission. Results The serum ALT and AST was higher in pMCAO group when compared with the control group (P〈0.05). The level of plasma endotoxin was significantly higher in the MCAO group than in the control group (P〈0.05). Both liver tissue and intestinal mucosal were suffered different damage that could be found under the light and electron microscopy. Conclusion We could establish the focal cerebral ischemia animal mode of hepatic stress injury and stress induced intestinal injury by adopting the method of Longa suture-oeculuded. Hepatic stress injury focal cerebral ischemia may be concerned with enterogenous GET.
