摘 要:目的 :明确FHIT基因蛋白产物 (Fhit)细胞内定位及分布情况 ,比较其在肾细胞癌 (RCC)组织、癌周及正常肾组织中的表达差异 ,探讨Fhit表达状态与RCC临床病理特征的关系。方法 :采用兔抗 Fhit多克隆抗体对 36例RCC的癌组织、2 0份相应癌周肾组织及 6例正常肾组织标本行免疫组织化学ABC法染色 ,结果以半定量方法记录分析。结果 :RCC患者术后五年随访过程中 ,复发及转移发生率分别为 16 7% (6 /36 )和 7 7% (10 /36 )。五年生存率为 5 5 6 % (2 0 /36 )。正常肾组织中特异性着色区主要分布在肾小管上皮细胞胞浆 ,呈弥漫性表达 ,少量位于肾小球内。癌组织中特异性着色区位于癌细胞胞浆 ,Fhit阳性率为 5 2 8% (19/36 ) ,癌组织中Fhit表达强度明显低于正常肾脏组织和相应癌周肾组织 (P <0 0 1)。Ⅲ、Ⅳ期癌组织Fhit表达显著低于Ⅰ、Ⅱ期者 (P <0 0 5 )。RCC组织Fhit失表达与患者预后差关系密切 (P <0 0 5 )。结论 :Fhit为一种细胞浆蛋白 ,正常肾组织中呈强阳性表达 ,绝大部分位于肾小管上皮细胞。癌组织中Fhit表达显著降低或失表达。Purpose:To elucide the subcellular localization and distribution pattern of Fhit protein, compare the difference between Fhit protein expression in tissues of RCC and surrounding carcinoma and normal kedney, explore the relationship between Fhit expression and clinopathological features of RCC, and prompt clinical and academic study of RCC. Methods:All paraffin sections were stained by multiclonal specific antibody (rabbit anti Fhit antibody). The slides were incubated in avidin biotin peroxidase complex (ABC) reagent (ABC kit, Vector). The results were recorded by semi quality method. Results:In 5 years follow up,6 cases (16.7%) recurred and 10 cases (27.7%) metastasis, survival rate was 55.6%(20/36). Positive stain were confined to the convoluted tubular epithelium and little glomeruli in normal kidney and surrounding cancerous tissue and in the cytoplasm of malignant cell in RCC. Fhit positive rate of RCC were 52.8% (19/36). Meantime, Fhit protein was expressed less extent in Ⅲ and Ⅳ grade (robson)tissue than in Ⅰ and Ⅱ of RCC ( P <0 05). Additionally, absent of Fhit protein was related to poor prognosis of RCC cases ( P <0 05). Conclusions:Fhit is a kind of cytosol protein. Fhit protein expresses intensely in convoluted tubular epithelium of normal kidney. In RCC, Fhit protein exhibits less extent or absent. The content of Fhit protein can predict the prognosis of RCC cases.
