端粒酶活性检测在乳腺癌诊断中的应用研究

摘　　要：目的　探讨乳腺肿块细针穿刺端粒酶活性检测在乳腺癌术前诊断中的临床应用价值。方法　对 85例乳腺肿块病人进行细针穿刺 ,对穿刺细胞分别用端粒重复扩增法 (TRAP)进行端粒酶活性检测和光镜细胞检查。以术后石蜡病理切片结果为最终标准 ,分别对两种检测方法在术前诊断乳腺癌方面的灵敏度、特异性和约登指数进行了比较。结果　术前端粒酶活性检测同穿刺细胞学相比 ,特异性分别为 73 9%和 82 6 % (P >0 0 5 ) ,差异无显著性 ;灵敏度分别为 91 9%和 6 4 5 % (P <0 0 5 ) ,约登指数分别为 6 5 8%和 47 1% (P <0 0 5 ) ,差异有显著意义。结论　乳腺肿块穿刺端粒酶活性检测在乳腺癌术前诊断中具有较大的应用价值。Objective\ To investigate the clinical value of telomerase activity by fine needle aspiration from breast masses for preoperative diagnosis of breast cancer.Methods\ 85 patients with breast masses were aspirated by fine needles in the study.The samples obtained by aspiration were detected with telomerase activity by telomeric repeat amplification protocol(TRAP)assay and observed with cytomorphology by microscope respectively.The sensitivity,specificity and Youden's index of the two detective methods in diagnosis for breast cancer were compared respectively while pathologic results from postoperative paraffin slice were considered as the final standard.Results\ Compared with cytomorphology by microscope,preoperative telomerase activity detection's specificity which is 73 9% versus 82 6%(P>0 05)had significant difference,but its sensitivity which is 91 9% versus 64 5%(P<0 05)and Youden's index which is 65 8% versus 47 1%(P<0 05)were significantly higher.Conclusion\ Telomerase activity detection of the samples obtained by aspiration from breast masses has practical value in preoperative diagnosis for breast cancer.
