准分子激光原位角膜磨镶术治疗高度近视并发症分析

摘　　要：目的　研究准分子激光角膜原位磨镶术 (LASIK)治疗近视的安全性和有效性。方法　回顾性分析LASIK治疗 463眼高度近视的并发症及预后。结果　术后裸眼视力与术前相比 ,75 2 9%达到或超过术前最佳矫正视力。术中并发症的发生率为不完全角膜瓣占 0 86% ,游离角膜瓣占 0 43 % ,角膜瓣偏位占 0 2 2 % ,角膜缘出血占 7 3 %。术后并发症的发生率为瓣下异物 2 8% ,屈光欠矫或过矫 9 8% ,层间角膜炎症 0 43 % ,皮质类固醇性高眼压 0 43 % ,角膜上皮植入 0 65 %。结论　准分子激光角膜原位磨镶术治疗高度近视安全有效 ,但需完善的设备和丰富的临床经验。Objective To evaluate the safety and efficacy of excimer laser insitu keratomileusis for high myopia. Methods 463 eyes of 237 cases which ranging from -6.00 D to -22.00 D undertook LASIK using SCMD Microkeratome and NIDEK EC-5000 excimer laser system and the comlications and prognosis were retrospectively analyzed. Results Postoperative uncorrected visual acuity increased significantly and 75.29% of patients achieved preoperative best corrected visual acuity of better. The peroperative complications included: incomplete corneal flap (0.86%), free cap(0.43%), decentered flap(0.22%), limbus hemaorrhage (7.3%). The postoperative complications included: particulate debris under the repositioned flap(2.8%),over or undercorrection ( 9.8% ),interface stromal infiltration(0.43%),steroidinduced ocular hypertension(0.43%0,epithelial interface ingrowth (0.65%).Conclusion LASIK for high myopia proved to be safe and effective, although it needed more improved microkeratome and experience.
