开放式冷冻外科治疗Ⅰ～Ⅱ级骨巨细胞瘤的光镜及电镜观察和远期疗效

摘　　要：目的 :为了降低骨巨细胞瘤刮除加植骨术后的五年复发率。方法 :对 8例Ⅰ～Ⅱ级骨巨细胞瘤病人作了刮除术后加开放式冷冻外科治疗 ,并对其中 5例作了术中冷冻肿瘤标本的光镜及电镜观察。结果 :术后对全部病例平均随访 10 .8年 ,均无瘤健存。 5例肿瘤标本经三次以上冻融期后 ,再经光镜及电镜观察证实肿瘤细胞遭受彻底破坏。结论 :刮除术后一般残腔壁存留肿瘤活细胞 ,造成术后复发 ,但加冷冻术可使残腔壁 2cm厚范围内的残存肿瘤细胞破坏 。Objective:To study the role of cryotherapy in decreasing the five-year recurrence rate of giant cell tumor of bone treated with curettage and bone grafting. Methods:Curettage plus open cryotherapy was carried out on 8 cases of Ⅰ～Ⅱ grade giant cell tumor of bone and the frozen specimens of 5 cases were observed with light and electron microscope. Results:All patients, followed postoperatively for an average of 10.8 years, were surviving. It was proved by light and electron microscopic observation that the tumor cells were completely destroyed after freeze-thaw more than 3 times. Conclusion:After curettage, there are some living tumor cells remaining on the wall of residual cavity and Cryotherapy can destroy the residual tumor cells existing within 2cm thickness of the wall of cavity. Cryotherapy can effectively decrease the postoperative recurrence.
