口外小切口下颌角肥大整形术研究

摘　　要：目的探讨口外入路下颌角肥大整形术的临床疗效并进行评价。方法在局部肿胀麻醉下，对52例下颌角肥大患者经口外唇形微创切口做直视下颌角弧形截骨和外板部分切除术。最后修整切口皮缘，用8-0尼龙线作切口精细美容缝合。结果本组52例患者术后切口愈合平整，无大出血、血肿、神经损伤和感染等并发症发生。术后12个月对29例患者随访，均获得理想的卵圆脸形，手术切口无瘢痕愈合。结论经口外微创切口行下颌角肥大整形术安全有效，切口无瘢痕愈合，有很好的实用和推广价值。Objective To investigate a simple, safe, effective and scar-free surgical technique for the correction of prominent mandibular angles by the external approach. Methods 52 cases of patients with prominent mandibular angles were chosen for this study. All surgeries were carried out by external approach under the local tumescent anesthesia. A lips-like mini invasive incision was designed and the prominent mandibular angle was exposed. Curved type osteotomies and lateral cortex osteotomies were easily underwent under direct eyesight. The incisions were sutured with 8-0 nylon thread according to microsurgical principles. Results All patients got successful surgeries under the local tumescent anesthesia without any complication. 29 patients were followed up one year post operation and they were all obtained ideal oven faces. The patients showed no scar in the incisions on the necks. Conclusion The correction of the prominent mandible angles by the external invasive incisions could obtain ideal oven faces without scar on the skin. The long term results prove that the surgical technique in this paper is safe, effective and worthy to be used widely.
