摘 要:目的 探讨肝细胞癌中 Bcl- 2、C- myc蛋白的表达 ,以及两者可能具有的协同性。方法 应用免疫组化双标记法检测肝癌组织、癌周组织及正常组织中两者的表达 ,并用图像分析系统加以定量分析。结果 Bcl- 2在三种组织中的表达量 (PU值 )分别为 2 1 .0 97± 3.460、8.635± 1 .384、3.0 59± 0 .1 2 8,C- myc分别为 36.70 2± 4.961、1 0 .585± 1 .2 63、1 .547± 0 .0 4 3,均具显著性差异(P <0 .0 1 )。在肝细胞癌无肿瘤被膜组中具有高 Bcl- 2表达 (P <0 .0 5) ,而在有肝硬化、具有门脉癌栓、无肿瘤被膜、高病理分级及高 TNM分期中具有高 C- myc表达 (P <0 .0 5)。高 Bcl- 2或(和 ) C- myc表达均与预后较差有关 (P <0 .0 5) ,经 Cox比例风险模型回归分析 C- myc为一独立的预后指标。结论 Bcl- 2与肝细胞癌发生有关 ,C- myc与肝细胞癌发生、发展有关 ,并可独立判断预后。Objective To evaluate the expression of Bcl 2、C myx protein in hepatocellular carcinoma(HCC),and the correlativity of the 2 oncogenes .Methods Immunohistochemical double staining technique (SABC) was used to detect the expression of the two genes in HCC tissues,liver tissues adjacent to cancer and normal liver tissues, and also analysed quantitatively by using the image system.Results It showed that the positive unit(PU) of Bcl 2 protein in 3 kinds of tissues were 21.097±3.460,8.635±1.384,3.059±0.128, respectively (P<0.01);the PU of C myc were 36.702±4.961,10.585±1.263,1.547±0.043, respectively (P<0.01).Exepression of Bcl 2 in tumor without capsule groups were significantly higher than that in the control(P<0.05).The higher positive staining expression of C myc were found in the case of cirrhosis and tumor thrombi in the portal vein(TTPV),as well as in the case of tumor without capsule, higher pathological grade and higher TNM stage(P<0.05).Higher expression of Bcl 2 or/and C myc were associated with shorter post operative surival(P<0.05).C myc expression can be regarded as an independent prognostic factor though Cox proportional hazad model.Conclusion Bcl 2 might be closely related to the occurrence of the HCC, C myc was associated with the occurrence and development of HCC, and also could evaluated the prognosis independently. 2 oncogenes have synergetic relationship in the process of occurrence of HCC.
