摘　　要：目的探讨^(125)I粒子治疗恶性肿瘤腹腔内种植转移灶的临床价值。方法根据放射性粒子植入计划系统(TPS系统),通过CT引导对12例患者共47个腹腔种植灶穿刺植入^(125)I粒子,术后2个月复查CT,回顾性分析临床症状及肿瘤大小变化、生活质量(QOL)评分,并观察并发症发生情况。结果 47个病灶均成功植入粒子,每个病灶植入10～60粒。术后2个月复查:肿瘤直径(1.5±0.9)较术前(5.1±2.3)em缩小,差异有统计学意义(P<0.05)。所有12例病例中,完全缓解(CR)3例,部分缓解(PR)8例,无变化(NC)1例,无进展(PD)病例。总有效率(CR+PR)91.7%。腹痛症状缓解率为83.3%,术后2个月QOL评分分别为(56.3±3.8)分较术前(42.2±5.7)分提高,差异有统计学意义(P<0.05)。无严重不良反应发生。结论 ^(125)I粒子植入是治疗恶性肿瘤腹腔内种植是一种短期内较为有效、安全的姑息性治疗手段。Objective To investigate the value of CT-guided ^125I seed interstitial implantation for the treatment of peritoneal metastasis. Methods CT-guided ^125I seed interstitial implantation was carried out on 12 patients with 47 peritoneal metastatic lesions. CT performed two months after the implantation and the clinical data were analyzed retrospectively. The clinical symptoms ,tumor size and quality of life before the treatment and two months after the treatment were compared and the complications were observed. Results ^125I seeds were implanted in 47 peritoneal metastatic lesions successfully with 10 to 60 seeds per lesion. At two months after the treatment, tumor diameters were significantly reduced from 5.1±2.3 em to 1.5±0.9 cm （P 〈 0.05）. Three patients had complete remission, eight had partial remission, one had no change and none had progressive disease. The overall treatment response rate was 91.7%. The improvement rate of abdominal pain symptoms was 83.3%. The quality of life score was significantly higher at two months after （56.3±3.8）than that before （42.2±5.7） treatment （P 〈 0.05）. There were no serious complications in any of the patients. Conclusion CT-guided ^125I seed interstitial implantation for peritoneal metastasis is an effective and safe treatment method.
