摘 要:目的:通过对2岁以下唇腭裂患儿采用磁共振成像(MRI)的形态学研究方法,调查低龄唇腭裂患儿高级听觉中枢可能的结构异常。方法:选取6月~2岁唇腭裂患儿18例及年龄相匹配的正常对照组婴儿18例,分别在1.5T磁共振成像仪上扫描采集脑结构数据,运用FreeSurfer软件重建脑结构并对各部分进行测量,对2组数据采用多元协方差分析方法,进行组间比较。结果:唇腭裂组患儿的左侧大脑灰质、左侧丘脑和左侧颞上平面(听觉皮层所在区域)的体积显著小于正常对照组,左侧颞上平面的厚度也显著低于正常对照组。右侧大脑半球、右侧丘脑及脑干的体积与正常对照组无显著性差异。结论:唇腭裂患儿的高级听觉中枢可能有异于正常儿童。这种异常也许构成了其中枢性听觉处理障碍(APD)的神经生物学基础。Objective: To investigate possible structural abnormalities of the auditory center of infants with cleft lip and/or palate. Method: Eighteen infants with cleft lip and / or palate, aged from 6 to 24 months, and eighteen normal infants were as control group. Morphological magnetic resonance imaging (MRI) measurements of brain structure of infants were recorded and analyzed using the "FreeSurfer" software package. The data were statistically analyzed using multivariate analy- sis of covariance (MANCOVA) between groups. Result: No significant group differences were found in right hemisphere, right thalamus and brain stem. However, infants with cleft lip and / or palate had statistically significantly smaller volumes of the left hemisphere, left thalamus and left superior temporal plane (auditory cortex), and significantly decreased thickness of the left superior temporal plane. Conclusion: The auditory center of infants with cleft lip and/or palate may differ from the normal group, and these structural abnormalities may constitute the neurobiological basis of central auditory processing disorders (APD).
