摘　　要：目的 :评价长春瑞宾 (诺维本 )单药治疗乳腺癌的疗效和毒副反应。方法 :多中心前瞻性临床研究。乳腺癌患者 2 0例 ,长春瑞宾 30mg/m2 第 1、5日静点 ,每 2 8天为一周期 ,每一患者 2周期。静注恩丹西酮 +地塞米松预防呕吐。结果 :OR6 4.8% ,其中CR10 .5 %。初次化疗患者OR高达 91.7% ,CR达 16 .7% ;再次化疗患者仅有PR2 8.6 %。乳腺原发灶OR83.3% ,CR16 .7% ;转移灶OR6 3.3% ,CR9.1% ,其中腋窝淋巴结转移灶OR6 2 .5 % ,CR12 .5 %。毒副反应以白细胞低下和呕吐最为常见 ,发生率分别为 89.7%和 5 1.3% ;其中Ⅲ度以上白细胞与中性粒细胞低下的发生率分别达 2 8.3%和 2 3.1%。Ⅲ度以上便秘的发生率为 13.2 %。结论 :长春瑞宾单药治疗初发乳腺癌疗效较突出 ,耐受性亦较好 ,重度骨髓抑制和便秘需引起足够重视。Objective: To evaluate the efficacy and toxicity of Vinorelbine as a single agent for breast cancer. Methods: Twenty patients with breast cancer were enrolled in this multi-center prospective study. Vinorelbine was given 30mg/m 2 intravenously on d1 and d5 of each cycle, 28 days a cycle and two cycles per patient. Antiemetic prophylactic treatment consisted of ondanseton with or without dexamethasone intravenously. Results: The overall response rate (OR) for the whole patients was 64.8% with 10.5% complete response (CR). In the chemotherapy naive patients, the OR was as high as 91.7% with a CR up to 16.7% ; while in those with prior chemotherapy, the PR was obtained only in 28.6% cases. For the primary tumors of the breast, an OR of 83.3% with a CR of 16.7% was documented; while for the metastatic lesions an OR of 63.3% with 9.1% CR was yielded, including 62.5% OR with 12.5% CR for the axillary lymph nodes. The most commomly side-effects of the agent were leukopenia and vomiting with frequencies of 89.7% and 51.3% respectively. Noticeably, leukopenia or neutropenia of no less than Ⅲ° was as frequent as 28.3% or 23.1% respectively. Constipation of Ⅲ° and Ⅳ° occurred in 13.2% cycles. Conclusion: Vinorelbine is a rather active single agent as primary chemotherapy for breast cancer with acceptable tolerance. Severe bone marrow suppression and constipation are the side-effects that deserve most attention.
