老年高血压病和冠心病胰岛素抵抗与肿瘤坏死因子的关系

摘　　要：目的　探讨老年高血压病 (EH)和冠心病 (CHD)患者的胰岛素抵抗与肿瘤坏死因子 (TNF -α)的相关性。方法　分别应用氧化酶法、放免法和酶联免疫法测定 3 5例EH患者、3 0例CHD患者及 2 2例健康老人的空腹血糖 (FBG)、血清胰岛素 (FINS)及TNF -α值 ,计算胰岛素敏感性指数 (ISI)。结果　两组患者均存在高胰岛素血症及胰岛素抵抗 ,而且 ,高血压组胰岛素抵抗程度较冠心病组严重。两组患者血清INS水平均与TNF -α呈显著正相关。结论　胰岛素抵抗程度与TNF -α水平升高密切相关 ,二者相互影响、相互促进 。Objective To investigate the relationship between insulin resistance and TNF-α in elderly patients with hypertension (EH) and coronary heart disease(CHD). Methods Fasting blood-gluocse(FBG), serum insulin level and TNF-α were measured in 35 EH patients, 30 CHD patients and 22 health elderly by oxidase method, RIA and ELISA, respectively. Insulin sensitivity index (ISI) was also calculated. Results Both groups showed insulin resistance and hyperinsulinemia, particularly in the EH group. The insulin level of both groups positively correlated with TNF-α.Conclusion The degree of insulin resistance corralates with high TNF-α level. Interaction between hyperinsulinemia and TNF-α may involved in the development of EH and CHD.
