摘 要:目的:研究骨髓中己糖激酶Ⅰ(HKⅠ)在慢性粒细胞白血病(CML)患者由慢性期(CP)向急变期(BP/BC,急髓变)转变过程中表达水平的变化。方法:选择慢性粒细胞白血病患者71例,其中实验组51例(包括24例急变期患者、27例慢性期患者),对照组20例。取实验组和对照组患者骨髓液,RT-PCR法检测HKⅠmRNA;免疫组织化学法检测HKⅠ在组织中的表达水平。用图像分析系统计算IOD值作为mRNA和蛋白表达量。结果:转录表达水平测定,CML急变期患者IOD值为0.836±0.105,慢性期患者为0.418±0.107,对照组为0.122±0.051,急变期患者明显高于慢性期患者,后者高于对照组,P<0.01;蛋白表达水平测定,CML急变期患者为1.230±0.187,慢性期为0.671±0.110,对照组为0.214±0.083,急变期患者明显高于慢性期患者,后者高于对照组,P<0.01。结论:转录表达水平及蛋白表达水平均显示,HKⅠ表达急变期、慢性期、对照组逐渐降低,提示HKⅠ可能参与了CML的发病机制,HKⅠ表达改变可能是促使CML由慢性期向急变期转变的另外一个条件,可联合其他指标一起作为病情监测的指标。OBJECTIVE:To detect the changes of the expression level of Hexokinasel I (HK I ) during the transi- tion from the chronic phase to blastic phase in the patients with chronic myeloid leukemia (CML). METHODS: This stud- y divided 71 selected cases into the experimental group (51 cases) with 24 cases with CML in the blastic phase and 27 ca- ses with CML in the chronic phase and the control group (20 cases). The bone marrow fluid extracted from the two groups were measured by HK I HK I mRNA was measured with RT-PCR and the expression level of HK I in the tissue was detected with immunohistochemistry. The value of IOD calculated by image analysis system was mRNA and the protein expression. RESULTS: At the transcriptional level,the HK I expression level in patients with CML in the blastic phase was 0. 836±0. 105,in the chronic phase was 0. 418±0. 107 and in the control group was 0. 122±0. 051,the HK I expression level in the blastie phase was higher than that in the chronic phrase and the HK I expression level in the chronic phrase was higher than that in the control group (P〈0.01). At the protein expression level,the expression level of HK I in patients with CML in the blastic phase was 1. 230±0. 187,in the chronic phase was 0. 671±0. 110 and in the control group was 0. 214±0. 083. The expression level of HK I in the chronic phrase was higher than that in the control group,and the expression level of HK I in the blastic phrase is higher than that in the chronic phrase (P〈0.01). CONCLUSIONS: The measured transcriptional expression level and the protein expression level both indicate that the HK I expression in the blastic phrase is higher than that in the chronic phrase and the HK i expression in the chronic phrase is also higher than that in the control group. This indicates thatthat HK I is necessary to sustain the malignantphenotype of the cells of the patients with the blastic phrase and that the change of the HK I expression is another factor that impels the transition of
