经静脉注射与经皮下注射超声造影剂检查乳腺癌前哨淋巴结的比较

摘　　要：目的探讨经静脉与经皮下注射超声造影剂SonoVue检查乳腺癌前哨淋巴结(SLN)的价值。方法对52例疑似乳腺癌的患者,先行经肘静脉注射的超声造影(CEUS);爆破造影剂后,经肿物周边皮下4个方向分别注射超声造影剂,观察并记录淋巴结的增强情况,用标记针标记检出的SIN。结果经肘静脉注射超声造影,检出转移性SLN的个数为7枚,检出率为12.3%。经皮下注射超声造影,检出SLN的40例中5例为乳腺良性肿瘤,检出率为41.6%;其余35例为不同分期的乳腺癌患者,检出率为87.5%;敏感度为88.9%,特异度为78.9%。结论经皮下注射较经静脉注射超声造影对乳腺癌前哨淋巴结的定性有明显的优势。Objective To discuss the effectiveness of intravenous and percutaneous CEUS in the detection of sen tinel lymph nodes in breast cancer. Methods Of 52 breast cancer clinical suspects, intravenous and percutaneous CEUS were performed in turns, the SLN has been recorded and marked by mark needle by observing the contrast en- hanced pattern of sentinel lymph node. Results By intravenous CEUS, we detected five patients with 7enhanced lymph node, with 12.5 %detection rate. By percutaneous CEUS, we detected forty patients with enhanced lymph nodes, with 41.6% detection rate for 5 benign cases. The rest thirty five patients were in different stages of breast cancer with 87.5 %detection rate. The total sensitivity of percutaneous CEUS was 88.9%, specificity was 78.9 %. Conclusions Compared with intravenous CEUS in qualitation of sentinel lymph node, percutaneous CEUS has apparent superiority.
