摘 要:目的探讨治疗肝内胆管结石的最佳方法。方法对52例肝内胆管结石患者在超声引导下经皮经肝胆管穿刺,其后扩张穿刺部位,建立人工窦道,胆管镜经人工窦道进入胆管取石。结果 52例穿刺62针,1例穿刺未成功,3例因脱管行第2次穿刺,5例因穿刺位置不理想行第2次穿刺,51例取石成功。结论介入性超声与胆管镜结合,可以达到微创治疗肝内胆管结石、保护肝功能的目的 ,本方法创伤小、效果显著。Objective The best treatment of intra-hepatic bile duct stone. Methods Ultrasound-guided percuta- neous transhepatic puncture was performed in 52 patients with intrahepatic bile duct calculus,Artificial sinus tracts were established after percutaneous transhepatic cholangiography, intra-hepatic bile duct stones were carried out by cholangioscopy through the artificial sinus tracts. Results Of 52 patients with percutaneous transhepatic cholangiogra- phy,51 patients were treated successfully, was put into effected 62 times, bile duct puncture was unsuccessful in one patient. 5 patients were punctured twice as disengaged of drainage tube or the location was ideal of puncture. Conclu- sions The combination use of interventional ultrasound and choledochoscopy was minimally inuasive to treat intra-he patic bile duct stones with liner protection. It was effective with the advantage of litlle trauma.
