摘 要:目的评价经股动脉微型导管药盒系统(miniature porl—catheler system.MPCS)植入治疗膀胱癌的疗效。方法16例膀胱癌患者先行双侧髂内动脉前干化疗栓塞,明胶海绵颗粒加弹簧圈栓塞非主要供血侧髂内动脉前干,主要供血侧髂内动脉前干用明胶海绵颗粒做适量栓塞后,经股动脉植入MPCS,导管留置于主要供血侧髂内动脉前干.术后定期经MPCS灌注化疔.化疗方案为吡柔比星、卡铂和丝裂霉素。结果16例患者均成功植入MPCS,经过52例次穿刺药盒灌注化疗后,肿瘤完全缓解(CR)3例,部分缓解(PR)11例.稳定(SD)1例.总有效率(CR+PR)为87.5%(14/6).其中3例PR患者结合适形放疗后2例肿瘤消失.1例忠者肿瘤明显缩小,经尿道膀胱肿瘤切除后继续经MPCS灌注化疗,术见肿瘤复发。全部患者经1个疗程的化疗后血尿消失.临床症状得到缓解,生质量明显提高。经留置MPCS灌注化疗的毒副反应轻微,无严重并发症发生。结论经股动脉微型导管药盒系统植入治疗膀胱痛的疗效满意,技术简便、安全,值得推广应用。Objective To evaluate the therapeutic efficacy of percutaneous femoral artery miniature port catheter system (MPCS) implantation for bladder cancer. Methods 16 patients with bladder cancer were treated first with bilateral anterior branches of inter nal iliae arteries chemotherapy and embolization, the non-blood side of the main anterior branches of internal iliac artery witb gelatin sponge particles and coil embolization, the main blood supply side of anterior branches of internal lilac artery with proper amount of gelatin sponge particles embolization,and then percutaneous femoral artery MPCS implantation through the catheter into the main blood supply side of the anterior branches of internal lilac artery, after that on a regular basis by the MPCS perfusion chemotherapy using pirarubiein, earhoplatin and mitomycin was done. Results MPCS in 16 patients were successfully implanted, after perfusion chemotherapy with a total of 52 times puncture MPCS, the tumors showed complete remission(CR),partial remission(PR)and sta-bilization of disease(SD) in 3 cases, 11 cases and 1 case, respectively, the total effective rate(CR+PR) was 87.5% (14/16) , of 3 ca-ses with PR after combined with conformal radiotherapy, the tumors disappeared in 2 cases and obvious reduction in one case and treated with transurethrai resection of bladder's tumors and continue through MPCS infusion chemotherapy, no tumor recurrerlce occurred. Hematuria disappeared,clinical remission and having better quality of life in all patients after treated by a course of infusion chemotherapy. The indwelling MPCS chemotherapy caused mild toxicity and no serious complications. Conclusion The effective-ness of infusion chemotherapy by percutaneous femoral artery MPCS implantation for bladder cancer is satisfied, this technology is simple ,safe and worth promoting.
