经皮穿刺氩氦刀冷冻联合支气管动脉栓塞化疗治疗非小细胞肺癌的疗效初探

摘　　要：目的探讨经皮穿刺氩氦刀冷冻联合支气管动脉栓塞化疗(BACE)治疗肺癌的临床效果。方法回顾性分析42例经氩氦刀冷冻联合BACE治疗的非小细胞肺癌患者的临床资料,通过联合治疗前后影像学动态观察及KPS评分变化的分析,探讨联合治疗方案的疗效。结果联合治疗后复查CT,发现肿瘤体积较治疗前明显缩小,27例患者(64.3%)肿瘤直径缩小>50%。治疗后1个月、6个月、12个月及24个月有效率分别为78.6%、73.8%、64.3%及57.1%;患者KPS评分较治疗前显著增高;BACE后,90.5%患者出现发热,61.9%患者出现胸痛、胸闷;氩氦冷冻治疗后,35.7%患者出现轻微胀痛感,33.3%患者出现少量胸腔积液,上述症状均自行消失。结论经皮穿刺氩氦刀冷冻联合BACE方法治疗非小细胞肺癌疗效明确、并发症少。Objective To investigate the therapeutic effect-of Ar-He cryoablation combined with bronchial artery chemoembolization（BACE） in non-small cell lung cancer（NSCLC）. Methods 42 NSCLC patients＇data were analyzed retrospectively. They were treated by Ar-He cryoablation combined with BACE. CT image and KPS were observed dynamically to evaluate the therapeutic effect. Results Turner minimized significantly,64. 3% minimized more than 50%. （CR ＋ PR）% of post-treat in 1 month,6month, 12 month and 24 month was 78. 6% ,73.8%, 64. 3% and 57. 1% respectively. KPS of patients rise significantly than pre-treatment. After BACE, slightly fever happened in 90. 5% patients,and pectoral discomfort happened in 61.9% patients. After Ar-He eryoablation,slightly pain happened in 35. 7% patients, and slightly pleural effussion happened in 33.3% patients. All symptoms disappeared automatically several days later. Conclusions Ar-He cryoablation combined with BACE supplies an optimistic alternative for those NSCLC patients, with little complications.
