摘  要:目的探讨玻璃体腔注射环孢素A(CsA)对早期糖尿病(DM)大鼠视网膜功能、组织结构及超微结构的影响。方法实验研究。SD大鼠经链脲佐菌素诱导建立DM动物模型。实验分为正常对照组、未注射药物组(DM组)、注射CsA干预组(DM+CsA组)、注射对照液干预组(DM+DMSO组),每组9只。于建模成功后4周进行玻璃体腔注射,注射后48h应用视网膜电图(ERG)检测视网膜功能。然后处死大鼠获取视网膜,应用光镜、透射电镜检测视网膜结构变化。运用单因素方差分析方法分析4组大鼠在不同刺激光强下ERG结果的差异,辅以事后检验进行ERG结果的各组间多重比较。结果不同刺激光强度下4组大鼠的ERGb波振幅间的差异有统计学意义(F=14.760—28.890,P值均〈0.01);当刺激光强度≥-0.35log cd·s·m^-2时,4组大鼠的暗适应ERGa波振幅间的差异亦有统计学意义(F值分别为12.510和15.500,P值均〈0.01)。与正常对照组(ERGa波:82.43±26.68,ERGb波:208.40±51.20)相比,DM组(ERGa波:39.71±7.61,ERGb波:92.20±24.42)和DM+DMSO组(ERGa波:37.63±17.25,ERGb波:93.11±22.50)大鼠暗适应时均有ERGa波、b波振幅下降(t=5.448—7.872,P值均〈0.05),DM+CsA组b波振幅(160.10±43.49)下降明显(t=3.299,P〈0.05),但a波(63.91±20.32)无明显变化(P〉0.05)。石蜡切片染色结果显示,与正常对照组相比,其他三组大鼠视网膜外核层疏松变薄、细胞超微结构紊乱呈凋亡表现。与DM组及DM+DMSO组相比,DM+CsA组暗适应时ERGa波、b波振幅下降程度轻(t=3.203—4.759,P值均〈0.05)、外核层疏松变薄程度轻、超微结构改变程度轻。结论DM大鼠病程早期即出现视网膜神经功能的损害,并有视网膜神经细胞超微结构改变。眼内注射CsA对大鼠视网膜功能有保护作用�Objective To investigate the electrophysiological and morphological changes of retinae after intravitreal injection of cyclosporin A ( CsA ) in experimental diabetic rats. Methods Experimental study. SD diabetic mellitus (DM) rat models were induced with intraperitoneal injection of streptozotocin. The rats were divided into four groups, with 9 rats in each group : the normal control ( group CON ), the diabetic rats with or without CsA intravitreal injection respectively( group DM + CsA and group DM), and the diabetic rats with dimethyl sulfoxide ( DMSO ) intravitreal injection ( group DM + DMSO ). The intravitreal injection of CsA or contrast solution was performed 4 weeks after the modeling. The retinal function of rats was examined by eleetroretinogram (ERG) 48 hours after the intravitreal injection. All rats were sacrificed and the structural changes of retina were observed by optical and transmission electron microscope. The datas of ERG including amplitudes and latencies of a-wave and b-wave were analyzed by One-way ANOVA and Bonferroni post hoe test. Results There was difference in ERG b-wave among the 4 groups under different stimuli light intensity ( F = 14. 760-28. 890, all P value 〈 0. 01 ). And there was difference in ERG a-wave among the 4 groups when the stimuli light intensity ≥- 0. 35 log c d· s· m^-2( F = 12. 510,15. 500, both Pvalue 〈 0. O1 ). Comparing to the group CON (ERG a-wave :82.43 ±26. 68, ERG b-wave :208.40 ± 51.20), the ERG a-wave and b-wave amplitudes of group DM (ERG a-wave:39. 71±7.61, ERG b-wave:92. 20 ± 24. 42) and group DM + DMSO ( ERG a-wave : 37.63 ±17.25, ERG b-wave : 93.11± 22. 50 ) ( t = 5.448 - 7. 872, all P value 〈 0.05 ), and the ERG b-wave of group DM + CsA ( 160. 10 + 43.39 ) ( t = 3. 299, P 〈 0. 05) were declined in dark adaptation,while the ERG a-wave (63.91±20. 32) of group DM + CsA had no difference ( P 〉 0. 05 ).
