摘 要:目的:比较口腔红斑和白斑处组织病理学诊断和上皮细胞内DNA倍体分析结果,研究口腔中、重度异常增生及癌变组织中异常体细胞及细胞核内DNA分布区的变化。方法:用小刷刷取口腔红斑或白斑处的上皮细胞,经固定后,涂片离心机制成1~2张玻片,行Feulgen染色做DNA倍体测定。同时在可疑处做活检行组织病理检查。比较病理和DNA倍体测定结果,同时测定口腔上皮细胞核内DNA分布区的变化。结果:46例病例中,病理证实为27例无异常增生、6例轻度异常增生、7例中度异常增生、5例重度异常增生和1例浸润癌。DNA倍体分析在无异常增生和轻度异常增生病例中未见DNA倍体异常;除1例外,另12例中、重度异常增生和浸润癌均可见DNA倍体异常。对12例口腔异倍体细胞定量测定发现5c(7±2.6)%、7c(11±4.3)%和9c(16±5)%细胞核内高DNA分布区明显较2c细胞(2±1.2)%增多(P﹤0.05)。结论:口腔中、重度异常增生和浸润癌的病例多可出现DNA倍体异常;DNA倍体异常细胞伴有核内高DNA分布区增大。Objective: To study aneuploid cells and nuclear DNA distribution in oral preeancer and cancer cases. Methods: Oral smears from 25 cases of erythroplakia and 21 cases of leukoplakia were collected by a brush and fixed. One to two slides per sample were made by a cytospin. Slides were stained by Feulgen solution for DNA ploidy analysis. Biopsios also were examined by a pathologist. Results: In total of 46 cases, 27 non--neoplastic le- sions, 6 mild dysplasia, 7 middle dysplasia, 5 severe dysplasia and I invasive cancer were diagnosed by a patholo gist. Expert of one cases, aneuploid ceils were detected in other 12 cases of middle, severe dysplasia and invasive cancer. DNA distribution area was also studied by a DNA image cytometer. High DNA area in nuclei of aneuploid cells, such 5c cells(7±2.6%), 7c cells(11±4.3%)and 9c cells(16±5%), were significantly increased than diploid cells(2±1.2%) (P〈0.05). Conclusion: Aneuploidy cells were detected in oral precancers and cancers. Nuclear high DNA area in aneuploid cells were larger than diploid cells.
