开窗减压术治疗牙源性囊肿的临床研究

摘　　要：目的：评价牙源性囊肿初期行开窗减压术治疗的临床疗效。方法：回顾2005年2月～2012年2月65例经开窗减压术治疗的牙源性囊肿病例，随访6个月～7年，进行临床、影像学及病理学检查，通过曲面断层片测量减压术前及术后病灶的长径变化，评价临床疗效，随访观察复发情况。结果：囊性病损减小约75．2％，平均减压时间约（8．3±4．2）个月，其中小于等于18岁的患者一般约为7．3个月，成人约11．3个月（P〈0．05），小于1Ocm。的囊肿平均减小率约为0．13，大于20cm。的囊肿平均减小率约为0．09，二者无显著性差异（0．0815），小于等于18岁者减小率约为0．15，成人约为0．08（P〈0．05）。1例牙源性角化囊性瘤方法，经二期手术治愈。结论：开窗减压术治疗牙源性角化囊肿是一种安全、可靠、有效的治疗方法。对于年轻患者其减压时间相对较短，但对于进展性牙源性囊肿，仍推荐行二期手术治疗。Objective To evaluate the clinical effectiveness of decompression as the initial treatment for odontogenic cysts. Methods 65 cases of odontogenic cysts were performed decompression from 2005.2 to 2012.2.AII the patients were followed up for 6 months to 7years.Clinical,imaging and pathological examinations were performed for evaluation of the results. The changes in size of cysts before and after decompression were measured on pantomogram; the clinical effect and the recurrence rate were evaluated. Results Decompression reduced lesion area by a mean of 75.2%. Mean decompression time was 8.3＋4.2months;it was7.3 months in patients ~〈18 years old and 11.3months in older patients （P〈0.05）.Mean rate of reduction was 0.13 in cysts〈10cm2 and 0.09 in cysts 〉20cm2 （0.0815）;by age,values were 0.14 in patients~〈18 years old and 0.08 in older patients （P〈0.05）.1 patient with odontogenic keratocyst recurred, and was healed by a secondary surgery. Conclusion Decompression Treatment of odontogenic cysts is a safe,reliable and effective method of treatment.A shorter decompression period is needed for young patients.For aggressive lesions, secondary definitive surgery is recommended.
