早期阴囊Paget病的临床诊治分析

摘　　要：目的提高对早期阴囊Paget病的认识及诊治水平。方法回顾性分析并总结2007～2011年间笔者医院收治的17例阴囊Paget病的临床症状、病变部位、病理、治疗方法及随诊情况。结果有15例患者术前经活检病理提示为阴囊Paget病后行手术治疗。17例行阴囊病灶扩大切除术,其中有2例行阴囊病灶扩大切除术+皮肤缺损处植皮术,1例行阴囊病灶扩大切除术+左腹股沟淋巴结清扫。17例行阴囊病灶扩大切除术后,其中有3例复发,再次行阴囊病灶局部扩大切除术,术后至今6～18个月未再复发。1例腹股沟淋巴结阳性者术后行放化疗,2年后死于全身转移。结论阴囊病灶局部扩大切除术+早期活检是治疗A期阴囊Paget病的主要方法,且预后良好。患者均需终身随访。Objective To improve the understanding, diagnosis and treatment of the early Paget＇s disease. Methods The clinical symptoms,diseased region, pathology, treatment and follow up results of 17 patients with scrotum Paget＇s disease from 2007 to 2011 in our hospital were retrospectively analyzed. Results There were 1S cases to carry out an operation after scrotal Paget＇s disease confirmed by preoperative biopsy. In the 17 patients received extensive resection of the scrotum, 2 patients received ski grafting and 1 patient received left inguinal lymph node dissection. 3 of 17 patients recurred and received extensive resection of the scrotum again, and did not recurred in 6 -18 months. 1 patient with inguinal lymph -node -positive received chemotherapy and radiotherapy treatment, and died from sys- temic metastases 2 years later. Conclusion The main method of treating the scrota1 Paget＇s disease in A stage is the extended resection of the scrotum and early biopsy of scrota1. The patients＇ prognosis is good. Follow - up is necessary in all patients＇ life.
