针刺通里穴对健康青年双侧大脑中动脉血流动力学影响的研究

摘　　要：目的:研究针刺通里穴对健康青年双侧大脑中动脉血流动力学的影响,探讨该穴在治疗脑血管疾病中的应用价值和安全性。方法:选取16名健康青年,男、女各8名,针刺左侧通里穴,运用TCD在针刺前10 min、进针时、行针时、出针时和出针后10 min分别观测双侧大脑中动脉收缩期最大血流速度(Vs)、舒张期最大血流速度(Vd)、平均血流速度(Vm)、搏动指数(PI)及阻抗指数(RI)。结果:在男性,双侧MCA的Vs、Vd、Vm在针刺后数个时间点与针刺前相比均显著变小,且左侧较右侧持续时间长;双侧MCA的PI和RI无显著变化。在女性,双侧MCA的Vs、Vd和Vm在各个时间点均无显著变化,仅右侧MCA的PI和RI在进针时显著变小。结论:针刺左侧通里穴对男性双侧MCA的血流速度具有负性累计效应。针刺左侧通里穴对女性双侧MCA的血流速度无显著影响,仅对右侧MCA的血流阻力有负性累计效应。针刺左侧通里穴对MCA的血流动力学的影响具有性别差异。Objective：We observed the effects of acupuncture at Tongli point on the hemodynamics of bilateral MCA in healthy youth,and then to clarify the use value and safety of Tongli point for treating CVD.Methods：We chose 16 young people,8 male and 8 female,acupunctured their left Tongli point,and then observed the Vs,Vd,Vm,PI and RI of bilateral MCA by TCD at the following five time points： 10min before acupuncture,inserting the needle,manipulating the needle,withdrawing the needle and 10min after withdrawing.Results：In male,Vs,Vd and Vm of bilateral MCA reduced at several time points in comparison with that before acupuncture,in addition,the changes lasted longer in the left MCA than that in the right;PI and RI of bilateral MCA had no remarkable change.In femle,Vs,Vd and Vm of bilateral MCA had no remarkable change,just PI and RI of right MCA reduced when inserting the needle compared with that before acupuncture.Conclusion：In male,there are negative accumulative effects on the blood speed in bilateral MCA when acupuncturing the left Tongli point.In female,there is no remarkable effect on the blood speed in bilateral MCA when acupuncturing the left Tongli point,but there are negative accumulative effects on the resistance of blood flow in the right MCA.There are differences in male and female in the effects of acupuncture at the left Tongli point on the hemodynamics of MCA.
