阴道超声在子宫内膜癌诊断中的应用价值

摘　　要：目的 :探讨阴道超声对子宫内膜癌的诊断价值。方法 :应用阴道超声探头对59例异常子宫出血的患者于诊断性刮宫术之前进行盆腔扫查 ,测量子宫内膜厚度 ,观察内膜的声像图改变特点以及评估肌间有无癌灶浸润 ,并与手术后病理学诊断进行对照。结果 :病理诊断子宫内膜癌52例 ,阴道超声诊断内膜癌43例。阴道超声诊断子宫内膜癌的灵敏度及特异度分别为78.85 %和71.43% ,阳性预测值95.35%。结论 :阴道超声作为一种无创性诊断手段 ,不仅可以作为评估异常子宫出血妇女的第一步常规检查 ,而且对子宫内膜癌的诊断具有临床应用价值。Objective:To evaluate the value of transvaginal ultrasonography in diagnosing endometrial cancer.Methods:The study included 59 patients with abnormal uterine bleeding.For each patient,transvaginal ultrasonography was performed before the curettage with the thickness of the endometrium measured,the ultrasonographic feature of the endometrium observed and the myometrial invasion evaluated.Then the results were compared with postoperative histopathological finding.Results:In the 59 patients,52 were histologically proved endometrial cancer,for which transvaginal ultrasonographic diagnosis was accurate in 43 patients with accuracy 77.85%,sensitivity 78.85%,specificity 71.43% and positive predictive value 95.35%.Conclusion:Transvaginal ultrasonography is a reliable and non invasive method in the diagnosis of endometrial cancer and assessment of myometrial invasion.
