摘　　要：探讨原发性脾脏肿瘤的临床表现和诊断、治疗原则。方法　回顾性分析2 1例原发性脾脏肿瘤的临床资料。结果　本组男 9例 ,女 12例。良性肿瘤 7例 ,恶性肿瘤14例。B超和CT是主要诊断方法。本组 2 0例均采用脾切除术 ,对脾脏恶性肿瘤则采用根治性脾切除 ,必要时辅以胰体尾切除术。恶性淋巴瘤术后辅以化疗与放疗。结论　原发性脾脏肿瘤的诊断关键在于提高警惕 ,并选用合适的影像学检查以确诊。Objective To investigate the diagnostic and therapeutic methods of primary splenic neoplasms(PSN). Methods The clinical data of 21 cases of PSN were analysed retrospectively.Results Of the 21 cases, 9 were males and 12 females. There were 7 cases of benign tumors and 14 malignant tumors in this series. Ultrasound and CT scan were the main diagnostic methods. Splenectomy was performed in all cases. For malignant splenic tumor, radical splenectomy and additional distal pancreatectomy should be done if neccessary. Adjuvant chemothepy and radiotherapy were be done for malignant lymphoma after operation. Conclusions The key points of the diagnosis of PSN are the vigilance to the diseases and choose appropriate imagic examination. Surgical treatment is the first choice of PSN.
