摘  要:目的研究宫颈上皮内瘤样病变Ⅲ级(CINⅢ)、宫颈癌和癌旁组织中T淋巴细胞、B淋巴细胞、巨噬细胞及诱生型一氧化氮合酶(iNOS)的表达及其意义。方法分别取20例正常宫颈上皮(NCE)、CINⅢ和宫颈浸润癌患者宫颈组织样本。用免疫组化方法检测其鳞柱状交界上皮间质内、损害性病变上皮组织内和浸润性癌组织内T淋巴细胞(CD3)、细胞毒性淋巴细胞(CD8)、B淋巴细胞(CD20)、巨噬细胞(CD68)和iNOS的表达及意义。结果与正常对照组和浸润性宫颈癌组相比,CD3、CD20、CD68及iNOS的阳性率在浸润癌癌旁组织中明显增高(P<0.05)。与浸润性宫颈癌组相比,CDCD+3和CDCD+20淋巴细胞更多存在于CINⅢ患者的组织内(P<0.05)。结论 T淋巴细胞、B淋巴细胞和iNOS在浸润性宫颈癌肿瘤旁组织间质内存在高表达。检测T淋巴细胞及B淋巴细胞亚群和iN-OS可用于宫颈癌及癌前病变的免疫监测。Objective To study the expression and significance of B and T lymphocytes, macrophages, and inducible nitric oxide synthase (iNOS) in the cervical stroma of women with grade III cervical intraepithelial neoplasia (CIN III ) or in the intratumoral and peritumoral tissues of women with invasive carcinoma. Methods Cervical tissue specimens were obtained from 20 each from control tissues, CIN Ⅲ and invasive carcinomas. The specimens were immunohistochemically labeled with antibodies to identify T lymphocytes ( CD3 ), cytotoxic lymphocytes ( CDs ), B lymphocytes ( CD20 ), macrophages (CDss) and iNOS. Results T lymphocytes, B lymphocytes, macrophages and iNOS were present more frequently in the stroma of peritumoral invasive tumors compared to the controls and intratumoral invasive cancer samples ( P 〈 0.05 ). CD3+ and CD20+ lymphocytes were present more frequently in CIN llI patients compared to the samples from patients with invasive tumor (P 〈 0.05). Conclusions High numbers of T and B lymphocytes, macrophages and iNOS expressing cells in the peritumoral stroma of the invasive tumors were observed. Cell migration appeared to be proportional to the progression of the lesion.
