进展期非小细胞肺癌NP化疗后放疗同步EP化疗疗效分析

摘　　要：目的回顾分析NP化疗后放疗同步EP化疗治疗进展期非小细胞肺癌疗效。方法采用NP化疗后放疗同步EP化疗治疗进展期非小细胞肺癌。化疗后放疗同步化疗组(A组)55例;化疗后单纯放疗组(B组)46例;两组患者接受2周期NP化疗后,分别行放疗同步EP化疗和单纯放疗。对近期疗效行卡方分析,生存率用Log rank检验。结果 A组与B组患者近期有效率分别为78.1%,69.5%,χ2=4.613,P<0.05;中位无进展生存期分别为9个月(4～19个月),7个月(3～17个月),P=0.043;1年总生存率分别为76.6%和65.3%,P=0.031;2年总生存率分别为49.2%和23.8%,P=0.036。中位生存时间分别为19个月和16个月,P=0.047。结论 NP化疗后放疗同步EP化疗治疗进展期非小细胞肺癌疗效优于NP化疗后单纯放疗。Objective To analyze retrospectively the clinical effect of NP induction chemotherapy fol- lowed by concurrent chemo-radiotherapy for advanced non-small cell lung cancer. Methods 101 patients with stage 111 non-small cell lung cancer were treated with NP induction chemotherapy, followed by concurrent chemo-radiotherapy versus radiotherapy alone. 55 patients（ A group）were treated with NP induction chemother- apy for two cycles, followed by concurrent EP chemotherapy and 3-dimentional conformal radiotherapy. 46 pa- tients（ B group）were treated with NP induction chemotherapy for two cycles, followed by 3-dimentional confor- mal radiotherapy alone. Conventional fractionation 3-dimentional conformal radiotherapy was performed using 6 MV x-rays with total dose 56 to 66 Gy. The clinical effects were evaluated by Chi-Square test. Survival time was / evaluated by Maplan-Meier Log rank test. Results The response rate of A group and B group was 78.1% and 69.5 %, respectively （ P 〈 0.05 ）. Median time to progression free survival in A group and B group was 9 months and 7 months,respeetively（P 〈0.05）. Overall survival of 1 year or 2 years in A group and B group was 76.6 % and 65.3 %, or 49.2 % and 23.8 %, respectively （ P 〈 0.05 ）. Median time to overall survival in A group and B group was 19 months and 16 months,respectively（ P 〈 0.05 ）. Conclusion The clinical effect of NP in- duction chemotherapy followed by concurrent EP chemotherapy and 3-dimentional conformal radiotherapy on advanced non-small cell lung cancer is better than chemotherapy alone.
