脂必泰治疗高血脂症的疗效及对脂肪肝、颈动脉斑块的影响

摘　　要：目的评价脂必泰胶囊治疗高血脂症的疗效及对脂肪肝程度、动脉内膜厚度及颈动脉斑块的影响。方法 80例高血脂症患者给予脂必泰胶囊240 mg/次口服,2次/d,分别于治疗前及治疗后3个月、6个月、12个月检测血脂、肝肾功能、肌酸激酶,并于治疗前及治疗后6个月、12个月行肝脏超声检查,治疗前及治疗后12个月行颈动脉超声检查。结果治疗3个月、6个月及12个月后,患者总胆固醇、三酰甘油、低密度脂蛋白胆固醇均显著下降,高密度脂蛋白胆固醇显著升高。治疗前后肝肾功能、肌酸激酶无明显变化。治疗6个月、12个月后脂肪肝程度较治疗前显著改善。治疗12个月后,颈动脉内膜厚度减小,血流速度增快,颈动脉斑块最大横截面积减小。结论脂必泰除能有效调节血脂外,还能有效改善脂肪肝程度、颈动脉内膜厚度和颈动脉斑块大小,且临床应用安全。Objective It is to evaluate the curative effect of Zhibitai on hyperlipidemia and its effect on fatty liver degree, carotid plaque and intima-media thickness. Methods 80 patients with hyperlipidemia were prospectively followed up,who re- ceived 240 mg of Zhibitai orally twice daily. Blood lipoproteins, myocardial enzymes, liver and renal functions were measured before treatment started, and after 3, 6 and 12 months of the treatment. Liver B -ultrasound detection was performed on be- fore treatment started, and after 6 and 12 months of the treatment. Carotid artery ultrasound detection was performed on before treatment started and after 12 months of the treatment. Results Plasma total cholesterol （FC） , plasma triglycerides （TG） and low-density lipoprotein cholesterol （LDL- C） were significantly decreased（ P 〈 0.01 ） , and high-density lipoprotein cholester- ol （HDL- C） was increased（ P 〈 O. 05 ）after 3,6 and 12 months of treatment . The degree of fatty liver was improved after 6 and 12 months of treatment. After 12 months of treatment, carotid intima-media thickness was decreased （P 〈 O. 05 ） , and flow velocities was increased （P 〈 0.05） , carotid artery plaque maximum cross-sectional area was reduced （P 〈 0.01 ）. Further- more, no significant changes of myocardial enzymes, hepatic and renal function test parameters were found. Conclusion Zhibi- tai not only can reduce plasma cholesterol levels, but also can improve the degree of fatty liver, reduce carotid intima-media thickness and carotid artery plaque size, and it is safe to use in clinic.
