摘  要:目的:利用血管回声跟踪技术(echo-tracking,ET)评价单纯脑梗塞患者颈动脉弹性。方法:选择临床确诊单纯脑梗塞患者50例作为病例组,又根据颈总动脉有无斑块及内膜增厚分为2个亚组,无斑块及内膜增厚者为病例组1,25例,有斑块或内膜增厚者为病例组2,25例,健康同龄志愿者50例作为对照组;ET技术检测颈总动脉,获得僵硬度参数:僵硬度指数(β)、压力应变弹性系数(Eρ)、动脉顺应性(AC)和脉搏波传导速度(PWVβ),两组间及两组内比较。结果:与对照组相比,病例组1,2的β,Eρ明显升高(P<0.05),AC和PWVβ差异统计学上不显著(P>0.05),但病例组1和2的AC总体上小于对照组,病例组1和2的PWVβ总体上大于对照组。病例组1和2的各个参数比较未见明显差异(P>0.05)。结论:脑梗死患者大动脉僵硬度显著增加,颈动脉斑块的有无不能明显影响动脉的弹性,用ET技术能准确显示大动脉僵硬度的变化,并能量化此种病变。Objective:To evaluate elasticity of the common carotid artery using echo-trackin (ET) technique in patients with simplex cerebral infarction. Methods: Fifty patients with simplex cerebral infarction (twenty-five with no intima-media thickness (IMT) changes and with no plaque as patients group 1; twenty-five with plaque or thickened IMT as patients group 2) and fifty healthy persons in the same age range were included. The immediate blood pressure of every person was got. We performed the ET exam off the bifurcation 2 cm of left common carotid artery of every person keeping away from the plaque and got the artery elasticity parameters: stiffness parameter (13), pressure-strain elasticity modulus (Ep) , arterial compliance (AC) . and one-point pulse wave velocity (PWV[3). Results: Compared with the control group, and Ep of the patients in group 1 and 2 were significantly higher( P 〈0.05), but AC and PWV of the patients in group 1 and 2had no significant difference ( P 〉0.05). But, AC of the patients in group 1 and 2 had the tendency of being lower than that of the control group, and PWV had the tendency of being faster. There were no significant difference in statistics of these parameters between patients in groupl and 2( P 〉0.05). Conclusion: Elasticity of the common carotid artery of the patients with simple cerebral infarction was significantly lower than that of the control group in the same age range. Plaque would not influence the stiffness of the carotid artery. ET technique could detect this kind of stiffness change in early stage and quantify in this kind of change.
