摘  要:目的研究表皮生长因子受体(EGFR)、血管内皮生长因子(VEGF)及增殖细胞核抗原(Ki-67)在胃肠间质瘤(gastrointestinal stromal tumors,GIST)中的表达,并分析其临床意义。方法采用免疫组织化学SP法检测138例GIST中EGFR、VEGF及Ki-67的表达情况,分析其临床意义及相互关系。结果 EGFR、VEGF及Ki-67在GIST中的阳性表达率分别为78.3%、61.6%和64.5%。EGFR的表达与肿瘤的直径(χ2=8.387,P=0.004)、包膜的完整性(χ2=4.779,P=0.029)、风险分级(χ2=17.182,P=0.000)和发生部位(χ2=13.362,P=0.000)相关;VEGF的表达与肿瘤的直径(χ2=7.891,P=0.005)、包膜的完整性(χ2=8.962,P=0.003)及风险分级(χ2=24.280,P=0.000)相关;Ki-67的表达与肿瘤的直径(χ2=7.626,P=0.006)、包膜的完整性(χ2=4.924,P=0.026)及风险分级(χ2=9.859,P=0.002)相关。三者的表达均与患者的年龄及性别无关。EGFR、VEGF和Ki-67经分析显示两两之间均呈正相关。结论 EGFR、VEGF及Ki-67可能参与了GIST的发生、发展过程,并在此过程中相互协调,提示三者可作为评价GIST危险程度的重要指标。Objective To analyze the expression and clinical significance of EGFR, VEGF and Ki-67 protein in gastroin- testinal stromal tumor (GIST). Methods Immunohistochemical staining of EGFR , VEGF and Ki-67 expression were conducted by streptavidin-peroxidase (SP) technique in 138 cases with gastrointestinal stromal tumor. The relationship between the expression and the clinicopathologic parameters was analyzed. Results The positive expression rate of EGFR, VEGF and Ki-67 in gastrointestinal stromal tumor were 78.3% , 61.6% and 64.5% respectively. EGFR expression was correlated with tumor diameter ( X2 = 8. 387, P = 0.004), integrity of tumor envelope ( X2 = 4.779, P = 0.029 ), risk grade ( X2 = 17.182, P = 0. 000 ) and tumor location ( X2 = 13. 362, P = 0. 000); VEGF expression correlated with tumor diameter (X2 = 7. 891, P = 0. 005), integrity of tumor envelope (X2 = 8. 962, P = 0. 003) and risk grade ( X2 = 24. 280, P = 0. 000) ; Ki-67 expression correlated with tumor diameter (X2 = 7.626, P =0. 006), integrity of tumor envelope ( X2 = 4. 924, P = 0. 026) and risk grade ( X2 = 9.859, P = 0.002). There was no relationship with age and gender. The correlation of EGFR, VEGF and Ki-67 expressions showed positive correlation in any of the two. Conclusion EGFR, VEGF and Ki-67 may play an important role in the pathogenesis and progress of GIST and accommodate each other. The expression of EGFR, VEGF and Ki-67 can help to judge the risk degree of GIST.
