摘 要:目的探讨不同病程的短暂性脑缺血发作(TIA)患者血清超敏C反应蛋白(hs-CRP)、血清尿酸水平的差异。方法 84例临床诊断TIA首次发作患者按病程分为A、B、C三组,即TIA持续60 min内缓解为A组,TIA超过60 min并反复发作,但无持久的神经功能缺损症状,影像学检查证实无脑梗死发生者为B组,C组为TIA起病后发展为脑梗死,在48~72 h经头颅CT或MRI证实有脑梗死发生。所有患者入院后检测血清hs-CRP及血清尿酸含量,28例健康体检者作为对照组,分析TIA的危险因素,并进行hs-CRP与尿酸水平的相关性研究。结果①四组研究对象hs-CRP和血尿酸水平差异均具有统计学意义,经多重比较C组hs-CRP和血尿酸水平均高于A组和B组(P<0.01),B组高于A组和对照组、A组较对照组高。②Logistic回归分析显示高尿酸血症、高hs-CRP、血脂异常、高血压、糖尿病为TIA的危险因素。③血清hs-CRP和血清尿酸水平呈正相关(r=0.71,P<0.01)。结论血清hs-CRP和尿酸水平升高可能是TIA的危险因素,临床检测血清hs-CRP和尿酸水平,可为TIA患者的病情评估提供可靠信息。Objective To explore the clinical significance of high sensitivity c-reactive Protein and serum uric acid in patients with transient ischemic attack.Methods 84 TIA patients were divided into three groups of A,B,C.Group A: TIA could be relieved in 60 minutes.Group B: TIA lasted for more than 60 minutes,but there were no neurological deficit and cerebral infarction,TIA.Group C.TIA deteriorated into cerebral infarction by CT or MRI within 48-72 hours.The high sensitivity c-reactive protein and serum uric acid level was determined in all patients within 24 hours separately.28 healthy subjects were used as control group.Results ①The high sensitivity c-reactive protein and serum uric acid levels of four groups were significantly different,.The above incices in group C were higher than those of group A and B(P 〈 0.01).②It showed that high levels of c-reactive protein and serum uric acid,hypertension,diabetes and dyslipidemia were the risk factors in patients with TIA with Logistic regression analysis.③ The correlative analysis of all groups demonstrated that high sensitivity c-reactive protein and serum uric acid had significantly positive correlation(r =0.71,P 〈0.01).Conclusion The increase of the level of high sensitivity c-reactive protein and serum uric acid are the probable risk factors of TIA.It can also provide useful information for patients with TIA by determining the level of high sensitivity c-reactive protein and serum uric acid.
