经颅磁刺激与阿立哌唑治疗利培酮所致高催乳素血症对照研究

摘　　要：目的 探讨低频重复经颅磁刺激与小剂量阿立哌唑治疗利培酮所致高催乳素血症的临床疗效和安全性.方法 将100例利培酮所致高催乳素血症的精神分裂症患者随机分为两组,每组50例,两组均维持原用利培酮治疗剂量不变,在此基础上研究组联合低频重复经颅磁刺激治疗,对照组联合小剂量阿立哌唑治疗.观察4周.于治疗前后采用阳性与阴性症状量表评定两组临床症状,同时检测血清催乳素水平,对检测结果进行对比分析.记录两组不良反应.结果 治疗后两组血清催乳素水平均较治疗前有显著下降（P＜0.01）;同期两组间比较差异均无显著性（P＞0.05）.研究组不良反应主要表现为轻、中度头痛,对照组主要表现为锥体外系反应,两组不良反应均较轻微,发生率差异无显著性（P＞0.05）.结论 低频重复经颅磁刺激与小剂量阿立哌唑均能显著降低利培酮引起精神分裂症患者的高催乳素水平,且疗效相当,安全性高.Objective To explore the efficacy and safety of low-frequency repetitive transcranial magnetic stimulation （rTMS） and aripiprazole in the treatment of hyperprolactinemia due to risperidone. Methods One hundred patients with risperidone-indueed hyperprolaetinemia were randomly divided into two groups of 50 ones each, both groups maintained originally used risperidone dosage, on this basis research group was plus low-frequency rTMS and control group plus small dose aripiprazole, they were observed for 4 weeks. Clinical symptoms of both groups were assessed with the Positive and Negative Syndrome Scale （PANSS） before and after treatment, serum prolactin levels detected at the same time and detection results contrastively analyzed. Adverse reactions of both groups were recorded. Results After treatment, the PRL levels of both groups lowered more significantly compared with pretreatment （P〈0.01）; there were no significant differences in contemporaneous group comparisons （P〉 0. 05）. Adverse reactions were mainly mild to moderate headache in research group and extrapyramidal symptoms in control group, those of both groups mild, and their incidences had no significant differences （P〉0.05）. Conclusion Both low- frequency rTMS and aripiprazole can lower high prolactin level of schizophrenics due to risperidone, have equivalent efficacy and higher safety.
