小梁切除术以穹隆或角膜缘为基底结膜瓣对泪膜的影响

摘　　要：目的探讨以穹隆或角膜缘为基底结膜瓣行小梁切除术对基底结膜瓣的影响。方法将行小梁切除术的青光眼患者90例(120眼)随机分为角膜缘组42例(60眼)和穹隆组48例(60眼)。角膜缘组以角膜缘为基底结膜瓣行小梁切除术,穹隆组则以穹隆为基底结膜瓣。观察2组结膜异物感、畏光感,检查结膜充血及分泌物情况,术后检测基础泪液分泌情况、泪膜破裂时间等。结果角膜缘组术后2、4周异物感、分泌物情况优于穹隆组,术后1、2、4周畏光感、结膜充血情况亦优于穹隆组;术后1d及1、2、4周基础泪液分泌量多于穹隆组,泪膜破裂时间长于穹隆组,差异均有统计学意义(P<0.05)。结论以角膜缘为基底结膜瓣行小梁切除术对泪膜的影响较小,可有效减轻青光眼的症状、体征及干眼症情况。Objective To investigate the influence of trabeculectomy with fornix or corneal limbus as basal conjunctival flap to the tear-film.Methods 90 cases of glaucoma patients（120 eyes） with trabeculectomy in our hospital were randomly divided into the fornix group of 48 cases（60 eyes） and corneal limbal group of 42 cases（60 eyes）,patients of fornix group underwent trabeculectomy with fornix for the basal conjunctival flap,patients of corneal limbal group were given cornea for the basal conjunctival flap.Observe conjunctival foreign body sensation,photophobia feeling of two groups and check the conjunctival congestion and secretion situation and test the based tear secretion,tear-film fracture time of two groups.Results The foreign body sensation and secretion situation of fornix group after 2 weeks,4 weeks was more obvious than that of the corneal limbus group;the photophobia feeling and conjunctival congestion of fornix group after 1 week,2 weeks,4weeks was more obvious than that of the corneal limbus group;the tear-film fracture time of fornix group after 1 day,1 week,2 weeks,4 weeks was shorter than that of the corneal limbus group;the foundation tear secretion of fornix group after 1 day,1 week,2 weeks,4 weeks of the fornix was less than that of the corneal limbus group,the differences were statistical significance（P〈0.05）.Conclusion Trabeculectomy with corneal limbus as basal conjunctival flap can effectively reduce the symptoms of glaucoma,signs,and dry eye.
