平板DSA的类CT软组织成像在肝癌小病灶检出中的应用

摘　　要：目的:探讨平板DSA的类CT软组织成像(XperCT,在飞利浦DSA中这项功能称为XperCT)在肝癌小病灶检出中的应用及价值。方法:43例拟肝动脉化疗栓塞术(TACE)的肝癌患者,术前均进行320排CT扫描,术中TACE之前先于肝固有动脉DSA造影,再从肝固有动脉注射造影剂做肝脏XperCT扫描,由3名影像科医师对320排CT、DSA、XperCT图像进行诊断,并对3种方法发现的直径≤3.0cm的小病灶分别进行分组计数并进行统计学分析:A组(直径<1.0cm),B组(1.0cm≤直径≤2.0cm),C组(2.0cm<直径≤3.0cm)。结果:43例肝癌患者共发现无重复计数的直径≤3.0cm肝癌小病灶125个,320排CT、DSA和XperCT分别发现51个、98个、123个,检出率分别为40.8%,78.4%,98.4%,三者统计学分析具有显著性差异(P<0.01)。其中A组检出肝癌小病灶分别为26个、66个、89个;B组分别为17个、23个、25个,C组分别为8个、9个、9个。结论:平板DSA的类CT软组织成像与320排CT和DSA相比,能检出更多的肝癌小病灶,具有很高的临床应用价值。Objective:To study CT like imaging of tissues of plane-detector digital substraction angiography in detecting small lesions of hepatocellular carcinoma.Methods:43 patients with HCC underwent 320-slice CT、DSA and XperCT befor TACE.Perform XperCT scan with injecting contrast media from proper hepatic artery.Images acquired with 320-clice CT,DSA and XperCT were diagnosed by 3 radiologists.There were 3 groups statistically analysed in small lesions of diameter≤3.0 cm of hepatocellular carcinoma:group A(diameter<1.0 cm),group B(1.0 cm≤diameter≤2.0 cm),group C(2.0 cm<diameter≤3.0 cm).Results:A total of 125 lesions(diameter≤3.0 cm) were diagnosed.320-slice CT detected 51 lesions and got 40.8% detection rate,DSA detected 98 lesions and got 78.4% detection rate,XperCT detected 123 lesions and got 98.4% detection rate.Statistical differences of detection were found between every two methods(P<0.01).The 125 lesions included respectively 26,66,89 lesions in group A,17,23,25 lesions in group B and 8,9,9 lesions in group C.Conclusion:CT like imaging of soft tissues of plane-detector digital substraction angiography can detect small lesions of hepatocellular carcinoma more than 320-slice,and have great clinical value.
