前列腺被膜支动脉血流相关参数与血清tPSA含量和fPSA/tPSA的关系

摘　　要：目的:探讨血清tPSA浓度和fPSA/tPSA比值与前列腺癌(PCa)血流动力学的变化关系。方法:选择经直肠超声(TRUS)引导前列腺组织学活检术和病理证实的前列腺疾病160例,其中PCa84例,前列腺增生症(BPH)76例(对照组),术前常规检测前列腺被膜支动脉的血流峰值速度(Vmax)、阻力指数(RI),用化学发光免疫分析检测血清tPSA、fPSA浓度,计算fPSA/tPSA的比值,进行对照分析。结果:PCa组血清tPSA浓度及被膜支动脉RI的平均值均高于对照组(P<0.001),而PCa组的fPSA/tPSA比值明显低于对照组(P<0.05);前列腺被膜支动脉RI值与血清tPSA浓度呈线性正相关(r=0.32,P<0.001)。结论:本文分析显示,前列腺被膜支动脉RI值与血清tPSA浓度密切相关。本文被膜支动脉RI≥0.80指标在预测PCa方面的准确性高于血清tPSA和fPSA/tPSA比值中任一单项指标,在临床超声诊断中具有较高的实用价值。Objective To explore the relation between the level of serum tPSA with the ratio of fPSA and tPSA and the blood stream dynamic change in prostate cancer.Methods To select 84 cases of prostate cancer and 76 cases of benign prostatic hyperplasia diagnosed with prostate tissue biopsy by TRUS and pathology,the blood flow Vmax,resistance index（RI）values of capsular artery of prostate gland with prostatic disease were tested and the levels of serum tPSA,fPSA were determined by chemiluminescence immunoassay,moreover,calculating the Ratio of fPSA and tPSA before operation and compared with corresponding values in the control group.Results The level of serum tPSA and the average RI value for capsular artery of prostate in the group of prostatic cancer was significantly higher than that of control group（P〈0.001）,and the ratio of fPSA and tPSA was lower than that of control group（P〈0.05）.There was a well correlation between RI value of prostate capsular artery and the level of serum tPSA（r=0.32,P〈0.001）.Conclusion The analysis showed that RI value of prostate capsular artery was closely related to the level of serum tPSA.The capsular artery RI value（RI≥0.80） was more accuracy than the level of serum tPSA or the Ratio of fPSA and tPSA in diagnosis of prostate cancer.The capsular artery RI value has a higher practical value in clinical ultrasound diagnosis.
