生大黄对重症急性胰腺炎并发肾损伤大鼠的治疗作用

摘　　要：[目的]探讨生大黄在大鼠重症急性胰腺炎(SAP)并发肾损伤时的治疗作用。[方法]将30只雄性SD大鼠随机分为假手术组、SAP组和治疗组,每组10只。采取4.5%牛磺胆酸钠胰胆管逆行注射制备大鼠SAP模型,治疗组在复制模型后即刻胃管内注入生大黄粉溶液。光镜下观察胰腺组织和肾组织的病理损害程度,测定肾脏组织湿干重比,检测各组大鼠血肌酐(Cr)和血尿素氮(BUN)含量,应用RT-PCR方法检测肾脏组织中肿瘤坏死因子-α(TNF-α)mRNA表达。[结果]治疗组大鼠肾脏湿干重比、Cr及BUN明显低于SAP组(P<0.01、<0.05、<0.05);假手术组未见TNF-αmRNA表达,与SAP组相比,治疗组TNF-αmRNA表达明显下降(P<0.05)。[结论]生大黄可以减轻胰腺组织和肾脏组织炎症程度,起到缓解病情、改善肾脏功能和促进修复的作用。[Objective]To investigate the effect of Rhubarb on severe acute pancreatitis （SAP） complicated with renal injury in rats. [Methods] Thirty SD rats were randomly divided into the sham operation group,SAP group and the treatment group（n： 10）. Rat SAP model was induced by injection of 4. 5% sodium taurocholate into the pancreatic duct. Rhubarb powder solution was injected into the stomach in the treatment group immediately after SAP model was built. Pathological changes of pancreas and kidney were observed with light microscope, kidney weight was measured for wet to dry weight ratio, and serum creatinine（Cr）and blood urea nitro- gen（BUN） were detected. RT-PCR was applied to detect tumor necrosis faetor-α（TNF-α）mRNA of the renal tissue. [Results]The wet to dry weight ratio of kidney, Cr and BUN in the treatment group were significantly lower than those in SAP group（P〈0.01; P〈0.05;P〈0.05）. No TNF-mRNA expression was detected in the sham operation group. Compared with SAP group,TNF- mRNA expression decreased obviously in the treatment group（P〈0.05）. [Conclusion]Rhubarb can ameliorate pancreatic and renal inflammation,thus alleviate the disease condition,improve renal function and promote the repairment.
