摘 要:目的探讨血管瘤患儿血清血管内皮生长因子(VEGF)、雌二醇(E2)及尿碱性成纤维细胞生长因子(bF-GF)水平的变化,以鉴别血管瘤和血管畸形,并为血管瘤临床分期提供依据。方法选择血管瘤及血管畸形患儿138例,其中增生期血管瘤(增生期血管瘤组)60例,消退期血管瘤(消退期血管瘤组)38例,血管畸形(血管畸形组)40例。采用ELISA法检测各组血清VEGF及尿bFGF水平,采用电化学发光法、使用自动免疫分析仪检测各组血清E2水平,并与正常对照组进行对照。结果增生期血管瘤组患儿血清VEGF、E2及尿bFGF水平显著高于消退期血管瘤组、血管畸形组、正常对照组(均P<0.01)。消退期血管瘤组、血管畸形组患儿血清VEGF水平与正常对照组比较差异均无统计学意义(均P>0.05)。消退期血管瘤组患儿血清E2水平显著高于血管畸形组、正常对照组(P<0.05)。消退期血管瘤组、血管畸形组患儿尿bFGF水平与正常对照组比较差异均无统计学意义(均P>0.05)。增生期血管瘤组女性血清VEGF、E2及尿bFGF水平均显著高于男性,但差异均无统计学意义(均P>0.05)。结论采用ELISA法检测血清VEGF、尿液bFGF及应用电化学发光法检测血清E2简便、易行、可靠,可为临床鉴别血管瘤和血管畸形,并为临床划分血管瘤分期提供依据。Objective To differentiate hemangiomas from vascular malformations and to provide the basis for the clinical stages of infantile hemangiomas by investigating the changes in serum levels of vascular endothelial growth factor(VEGF) and estradiol(E2) and urinary levels of basic fibroblast growth factor(bFGF).Methods A total of 138 cases of infantile hemangiomas and vascular malformations,selected in the among including 60 cases of proliferative hemangiomas(proliferative hemangiomas groups),38 cases of involuting hemangiomas(involuting hemangiomas groups) and 40 cases of vascular malformations(vascular malformations group).Serum VEGF and urinary bFGF levels were determined by ELISA and serum E2 levels were measured by electrochemiluminescence assay with automatic immune analyzer(Germany Roche Diagnostics GmbH) and compared with normal control group.Results Compared with proliferative hemangioma group,the levels of serum VEGF and E2 and urinary bFGF significantly decreased in involuting hemangioma group,vascular malformation group and normal control group(all P〈0.01).Compared with involuting hemangioma group,serum E2 levels obviously decreased in vascular malformation group and normal control group(P〈0.05).The levels of serum VEGF and urinary bFGF in control group were not significantly different from those in involuting hemangioma group and vascular malformation group(all P〉0.05).The levels of serum VEGF and E2 and urinary bFGF in female infants were higher than those in male infants,but the differences were not significant(all P〉0.05).Conclusion ELISA analysis of serum VEGF and urine bFGF levels and electrochemiluminescence assay for serum E2 detection are simple,easy and reliable in differentiating hemangiomas from vascular malformations and provide the basis for the clinical stages of infantile hemangiomas.
