高龄白内障合并糖尿病患者围手术期临床观察

摘　　要：目的观察高龄(80岁以上)糖尿病患者白内障围手术期的血糖变化规律、并发症及疗效。方法选取2010年1月至2011年12月期间在我院接受白内障手术的高龄糖尿病患者30例(30只眼),均采用白内障超声乳化吸除联合后房型人工晶状体植入术,观察手术前后血糖、视力及并发症。结果手术当日午餐前、晚餐前、睡前平均血糖均较术前上升,与术前血糖比较差异均有统计学意义;所有患者术后视力较术前均有不同程度提高,差异有统计学意义。结论高龄糖尿病患者施行白内障超声乳化联合后房型人工晶状体植入术疗效肯定,手术后当天血糖升高明显。适当干预后,可减少并发症的发生。Objective To observe the blood glucose variation, complications, and curative effect in advanced age diabetic patients with cataract surgery. Methods 30 patients with diabetes （30 eyes） from Jan, 2010 to Dec, 2011 in our hospital were reviewed. Phacoemulsification and posterior chamber intraocular lens implantation were performed in all pa- tients. Blood glucose ,vision before and after operation and complications were observed. The statistical method was per- formed by SPSS13.0 statistical software with t test and χ^2 test. Results On the day of operation, mean blood glucose be- fore lunch, dinner, before bedtime decreased as compared to preoperative rise. The preoperative blood glucose had signifi- cant differences. Postoperative visual acuity in all patients was improved to different degrees. The difference was statistical- ly significant. Conclusion Phacoemulsification and posterior chamber intraocular lens implantation is effective in advanced aged diabetic patients. On the day of operation, the blood glucose increased significantly. Appropriate intervention can re- duce complications.
