CIP2A在喉癌组织中的表达及其临床意义

摘　　要：目的探讨蛋白磷酸酶2A的癌性抑制因子(cancerous inhibitor of protein phosphatase 2A,CIP2A)在喉癌组织中的表达及其与临床病理特征的关系和对预后的影响。方法采用免疫组化方法检测61例喉癌标本(包括40例对应癌旁正常组织),9例不典型增生和8例喉乳头状瘤中CIP2A的表达,结合临床病理资料进行分析。结果 CIP2A在正常癌旁组织中低表达或不表达,在喉鳞状细胞癌中的表达比在喉乳头状瘤和不典型增生中明显增高(P<0.05)。61例喉癌中,CIP2A阴性表达11例(18.0%),弱阳性13例(21.3%),阳性25例(41.0%),强阳性12例(19.7%),其阳性程度与喉癌的年龄、性别、吸烟和嗜酒无关(P>0.05),而与肿瘤的分级分期、淋巴转移、组织分化程度和预后相关(P<0.05)。喉癌中CIP2A表达越高,其预后越差,高表达组总生存率和无瘤生存率低于低表达组(P<0.05)。结论 CIP2A参与喉癌的发生、发展过程,对评估喉癌预后有重要作用,可能成为喉癌治疗的有效分子靶标之一。Objective To determine the role of CIP2A in the development and progression of laryngeal squamous cell carcinoma and investigate its clinical significance. Methods Expression of CIP2A was examined with immunohistochemistry in 61 tissue samples of pathologically identified laryngeal squamous cell carcinoma and their 40 adjacent normal tissues, 9 atypical hyperplasia and 8 laryngeal papilloma. The relationship between CIP2A with the clinicalpathological characteristics and the effect of CIP2A expression on prognosis were analyzed. Results The expression of CIP2A was significantly higher in laryngeal squamous cell carcinoma than atypical hyperplasia and laryngeal papilloma （P 〈 0. 05）. CIP2A immunoreactivity, tested in 61 laryngeal cancer tissues, was negative in 11 cases （18.0%）, weakly positive in 13 cases （21.3%）, moderately positive in 25 cases （41.0%）, and strongly positive in 12 cases （ 19.7% ）. The expression of CIP2A was not related with age, sex, smoking and alcohol assumption. Whereas, there were significant correlations of CIP2A with T stage, clinical stage, lymph node metastasis, and histological differentiation （P 〈 0. 05）. High CIP2A expression predicted poor survival, and was associated with shorter overall-survival and disease-free survival. Conclusion Over-expression of CIP2A might be involved in the development and progression of laryngeal squamous cell eareinoma. CIP2A may serve as a useful prognostic marker and therapeutic target for laryngeal squamous cell carcinoma.
