梅尼埃病患者耳鸣疗效分析

摘　　要：目的分析梅尼埃病患者药物治疗前后耳鸣疗效与梅尼埃病分期．耳鸣病程分期、眩晕疗效及听力疗效等的相关性。方法24例（29耳）梅尼埃病患者给予药物治疗膜迷路积水，并进行为期6个月的随访。29耳均行纯音测听，了解患者治疗前后6个月最差听力损失情况，以分析梅尼埃病临床分期、耳鸣病程分期、眩晕疗效、听力疗效与耳鸣疗效的关系。结果入选的24例（共29耳）患者中，梅尼埃病早期10耳，中期19耳，晚期0耳。耳鸣分期：急性7耳，亚急性3耳，慢性19耳。耳鸣疗效：痊愈9耳，显效7耳，有效6耳，无效7耳。眩晕疗效：完全控制9耳，基本控制14耳，部分控制3耳，未控制0耳，加重3耳。听力疗效：显效3耳，有效4耳，无效14耳，恶化8耳。结论通过药物可控制梅尼埃病眩晕发作，但该药物治疗耳鸣的疗效与梅尼埃病分期。耳鸣病程分期、眩晕疗效及听力疗效之间无相关性。objective To analyze the effects of medication on the tinnitus of patients with Meniere＇s disease and the correlation between treatment outcome and the stage of M6ni6re＇s disease, the duration of tinnitus and the treatment outcome of vertigo and hearing loss. Methods The medical treatment was performed in 24 patients （29 ears） with Meniere＇s disease and the follow-up duration was 6 months. The pure-tone audiometry was carried out in all patients to evaluate the worst level of hearing loss before the treatment and during the follow-up period respectively. Results Of the 29 ears with Meniere＇s disease, there were 10 ears at the early stage, 19 ears at the mid-stage and no ear at the late stage. There were 7 ears with acute tinnitus, 3 ears with subacute tinnitus and 19 ears with chronic tinnitus. As for the tinnitus treatment outcome, 9 ears were cured, 7 ears markedly effective, 6 ears effective and 7 ears ineffective. As for the treatment outcome of vertigo, 9 ears were completely controlled, 14 ears generally controlled, 3 partly controlled, no ear uncontrolled and 3 aggravated. As for the treatment outcome of hearing loss, 3 ears were significantly effective, 4 ears effective, 14 ears ineffective and 8 ears aggravated. Through statistical analysis, there was no correlation between the treatment outcome of tinnitus and the stage of Meniere＇s disease, stage of tinnitus and the treatment outcome of vertigo and hearing loss. Conclusion The vertigo in patients with Meniere＇s disease can be controlled by medical treatment. The treatment outcome of tinnitus may be not correlated with the stage of Meniere＇s disease, the duration of tinnitus and the treatment efficacy of vertigo and hearing loss.
