摘 要:目的探讨减重支持系统训练对缺血性脑卒中偏瘫患者步行功能、运动功能及日常生活能力的影响。方法 65例符合入选标准的脑卒中偏瘫患者随机分为治疗组(31例)和对照组(34例)。两组均进行常规康复疗法训练,治疗组在此基础上结合减重支持系统训练,分别于治疗前、治疗后4周、8周末使用功能性步行量表(FAC)及步行速度、步长等指标进行步行功能评价,简式Fugl-Meyer运动功能量表(FMA)进行运动功能评价,改良Barther指数(MBI)进行日常生活能力评价。比较训练不同时间后偏瘫患者步行功能、运动功能及日常生活能力恢复程度的差异性。结果治疗前两组各项指标比较均无明显差异(P>0.05),训练4周后治疗组FAC评分、平均步行速度、平均步长及FMA分值均较对照组明显提高(P<0.05),MBI两组比较无明显差异(P>0.05),8周后治疗组FAC评分、平均步行速度、平均步长及FMA分值均较对照组明显提高(P<0.01),治疗组MBI较对照组前明显提高(P<0.05)。结论缺血性卒中后使用减重支持系统训练可显著改善偏瘫患者步行功能、运动功能及日常生活能力。Objective To study the effects on the walking function, movement function and the ability of daily life to the body weight support treadmill training for ischemic stroke patients with hemiplegia. Methods 65 cases for ischemic stroke patients with hemiplegia were divided into the treatment group (31 cases) and the con- trol group (34 cases). Both groups were given to regular rehabilitation therapy training, and the treatment group based on this body weight support treadmill training. Respectively, before treatment, and 4 weeks, 8 weeks after treatment, functional ambulation category (FAC) and walking speed, step length etc were used to access the walking function. Fugl-Meyer Assessment (FMA) to access movement function, Modified Barther index (MBI) to access the ability of daily life. Compared the differences of the walking function, movement function and the abili- ty of daily life after the different time of the body weight support treadmill training. Results The two groups each index for comparison were not significantly difference ( P 〉 O. 05), training after four weeks treatment group FAC score, average walking speed, average step length and FMA which was obviously improve the score with the con- trol group (P 〈 0. 05 ), BI index there were no significant differences between the two groups (P 〉 0. 05 ), 8 weeks after treatment group FAC score, average walking speed, average step length and FMA which is obviously improve the score with the control group (P 〈 0. 01 ), the treatment group BI index was significantly improve the control group (P 〈0. 05). Conclusion Acute ischemic stroke using weight training system support can improve the patients with hemiplegia walk function, movement function and daily living skills.
