PAX2在卵巢浆液性腺癌中的表达及在MDACC分级系统中的应用价值

摘　　要：目的探讨PAX2在卵巢浆液性腺癌中表达的意义,及其在二级组织学(MDACC)分级系统中的应用价值。方法回顾性研究55例经手术治疗的卵巢浆液性腺癌患者的临床病理资料,并结合临床病理参数进行统计学分析。运用免疫组化技术观察PAX2在卵巢浆液性腺癌中的表达水平,分析表达结果与组织学分级和临床病理参数之间的关系。结果 PAX2阳性表达定位于细胞核,阳性率为40%。PAX2的表达水平在肿瘤的复发、pTNM分期间有显著差异(P<0.05),Kaplan-Meier检验显示pTNM分期、肿瘤复发、MDACC分级、PAX2表达分别与预后有关(P<0.05)。Kappa检验显示PAX2表达水平与MDACC分级水平一致性较好(κ=0.773,P<0.01)。结论PAX2可以作为判断MDACC分级的辅助手段,其低表达或不表达对诊断MDACC高级别可信度较高。Objective To discuss the significance of the expression of PAX2 and to evaluate the value of PAX2 in judging MDACC grading system.Methods The clincopathological characteristics of 55 patients of ovarian serous adenocarcinoma by operative treatment were reviewed retrospectively.All the cases were chosen for immunohistochemical staining with PAX2 protein,and the correlation of the immunostaining with histologic grading and clinical pathological parameters was analyzed.Results The positive rate of PAX2 was 40.0%,and the positive expression of PAX2 and Ki-67 localized in the nucleus.There was statisticall significance between the expression of PAX2 with pTNM stage,recurrence and MDACC grading system（P〈0.05）.By the following-up of 55 cases,Kaplan-Meier analysis showed that pTNM stage,recurrence,MDACC and the expression of PAX2 were all related to the prognosis（P〈0.05）.The expression level of PAX2 had better consistency with MDACC grading system（κ=0.773,P=0.000）.Conclusions PAX2 expression may be meaningful in assisting MDACC grading;the week or negative expression of PAX2 may indicate a high grade cancer.
