突变型p53和凋亡抑制蛋白Livin在胃癌组织中的表达及其相关分析

摘　　要：目的:探讨突变型p53蛋白和凋亡抑制蛋白Livin在胃癌组织中的表达和临床意义以及二者的相互关系。方法:收集48例胃癌石蜡标本及20例癌旁正常组织标本,采用免疫组化法检测p53与Livin蛋白的表达,并分析不同临床、病理因素(性别、年龄、肿瘤大小、浸润深度、分化程度、淋巴结有无转移、临床分期)患者p53与Livin蛋白的阳性表达率。结果:胃癌组织中p53、Livin蛋白阳性表达率分别为50.0%、37.5%,两者在20例正常胃组织中无1例阳性表达,差异均有统计学意义(P<0.05);≥60岁、<60岁患者Livin蛋白阳性表达率分别为54.2%、20.8%,差异有统计学意义(P=0.017)。除此之外,其他各临床、病理因素患者胃癌组织中Livin、p53蛋白表达差异均无统计学意义(P均>0.05);p53和Livin蛋白在胃癌组织中的表达存在正相关(χ2=8.888,P<0.05,r=0.395)。结论:p53和Livin蛋白在胃癌组织中特异性高表达,可作为诊断指标和治疗靶点,二者在胃癌发生、发展中相互影响。≥60岁胃癌患者Livin表达明显升高,提示凋亡抑制是老年胃癌患者重要的发病原因。Objective： To explore the expression of p53 protein and Livin in gastric cancer,its clinical significance and relationship between the two.Methods： A total of 48 gastric cancer paraffin specimens and 20 normal tissue samples were collected and expression of p53 protein and Livin were evaluated with immunohistochemical method.Correlation between expression rate of p53 protein and Livin and clinical type,pathological characters were also analyzed.Results：The expression rate of p53 and Livin was 50.0% and 37.5%,but neither of them was positively expressed in healthy gastric tissues,showing significant difference between the two（P〈0.05）.Their expression rate in patients at 60 years or younger than 60 years were 54.2% and 20.8%（P=0.017）.No significant difference in expression rate of p53 protein and Livin was observed between gastric cancer of different clinical type or with different pathological characters（P〉0.05）.Positive correlation was observed between expression of p53 protein and Livin（χ2 =8.888,P〈0.05,r=0.395）.Conclusions：Specific expression of p53 and Livin can be used as diagnostic index and treatment target.The increased expression of Livin in patients older than 60 years suggests that inhibition of apoptosis play important role in elderly patients.
