临床药师对1例晚期胃癌患者的药学监护

摘　　要：目的:确保晚期胃癌患者用药的安全性和有效性。方法:临床药师通过对1例晚期胃癌患者的药学监护,结合患者的基础情况,将临床治疗中药物的选择、药物可能出现的不良反应、相互作用作为切入点。采取协助临床医师制订治疗方案、对患者进行用药教育、药物咨询等方式实施药学监护。结果与结论:患者顺利完成本周期治疗。通过实施药学监护,协助临床医师优化了治疗方案,提高了患者的用药依从性,保证了药物治疗的疗效,降低了药品不良反应的发生率。OBJECTIVE：To ensure the safety and effectiveness of drug use in patient with advanced gastric cancer.METHODS：Clinical pharmacist provided pharmaceutical care for 1 patient with advanced gastric cancer,and took selection of drugs,potential ADR and interaction as breakthrough point based on general information of patients.Clinical pharmacist provided pharmaceutical care by assisting clinical physicians to formulate therapeutic scheme,offering medication education and drug consultation,etc.RESULTSCONCLUTION：The treatment course has been completed smoothly.The compliance of patients,therapeutic efficacy of drug and the incidence of ADR have been improve by providing pharmaceutical care and assisting physicins to formulating therapeutic scheme.
