摘 要:目的:通过钼靶X线、免疫组化染色观察,探讨乳腺癌的VEGF、C-erbB-2表达与钼靶X线征象之间的关联性。方法:选取我院经过乳腺钼靶X线片诊断和临床病理证实、资料完整的乳腺癌病例50例,对其术后标本进行免疫组织化学染色并判断这些癌基因、免疫因子的表达情况。将乳腺癌钼靶X线表现中的肿块、恶性钙化、毛刺及腋下转移性淋巴结与VEGF、C-erbB-2表达情况进行对比分析。结果:50例乳腺癌病例中钼靶X线表现为肿块37例(74%),恶性钙化23例(46%),毛刺19例(38%),有腋下转移性淋巴结14例(28%);VEGF阳性表达者24例,C-erbB-2表达阳性者14例;肿块组、毛刺组、有腋下转移性淋巴结组VEGF表达阳性率高于无肿块组、毛刺组、腋下转移性淋巴结组(P<0.05);恶性钙化组、毛刺组、有腋下转移性淋巴结组C-erbB-2表达阳性率明显高于无恶性钙化组、毛刺组、腋下转移性淋巴结组(P<0.05)。结论:说明乳腺癌的钼靶X线上某些重要征象能反映肿瘤的分化程度及肿瘤预后,为临床制定治疗方案、疗效评价提供一定理论依据。Objective:To investigate the relationship of the features of breast cancer on molybdenum target radiograph with the molecular biology markers of VEGF and C-erbB-2.Methods:Fifty patients with breast cancer underwent initial diagnosis with molybdenum target radiography and confirmed pathologically in our hospital.Immunohistostaining was performed on the postoperative specimens to detect the expression of the cancer gene and immunologic factors.The mass,malignant calcification,veining and axillary lymph node metastases exposed on the mammographic images were compared with the expression of VEGF and C-erbB-2,and the data were summed up.Results:Of the 50 breast cancer cases,mammography revealed the mass in 37(74%),malignant calcification in 23(46%),veining in 19(38%) and axillary metastatic lymph nodes in 14(28%).VEGF and C-erbB-2 were positively expressed in 24 and 14,respectively,of the 50 cases.The positive rate of VEGF expression complied with the mammographical imaging signs of mass,veining and axillary metastatic lymph node was higher than that of failure of exposure of those elements(P〈0.05),and the positive expression rate of C-erbB-2 was also higher on the mammographs exposed malignant calcification,veining and axillary metastatic lymph node as compared with those of absence(P〈0.05).Conclusion:The findings of molybdenum target X-ray may,to a certain extent,reveal the tumor differentiation and be used to make an estimation of the prognosis,program the therapeutic regime and evaluation of the treatment effects.
