摘 要:目的通过让中风患者完成冲热饮的任务从而评估虚拟环境在康复治疗中的作用和价值。方法选取50例入住瑞安市人民医院康复科进行康复治疗的中风患者参与到实验中。患者在冲热饮过程中的表现与其神经系统损坏程度息息相关。将患者在虚拟环境以及真实生活中冲热饮过程中所犯的错误相比较,从而准确地评估神经系统损害,再根据录像把患者所犯的错误表现进行归类。结果患者在真实世界与虚拟环境的表现成绩不是非常关联(ρ=0.30,P<0.05)。两种环境的表现成绩都与年龄、日常生活活动(ADL)量表(Bartherl指数)评分、精神状态检查评分以及视觉空间功能有关,真实世界中冲热饮的表现只与臂力和顺序排列能力有关,而虚拟环境的表现只与语言功能和练习程度有关。无论在真实世界或是虚拟环境,参与者在冲热饮这个任务过程中都会犯各种错误。结论尽管虚拟环境可用于中风患者的康复治疗,但同时也存在很多困难,对康复治疗提出了很多挑战。所以虚拟环境也许不会像原先所设想的一个比较有效的康复手段。还需研究探索出更多的虚拟环境任务来评估其对于中风患者康复的作用。Objective To evaluate the role and value of the virtual environment through the stroke patient completing the task of making a hot drink. Methods Fifty stroke patients undergoing rehabilitation in our hospital were involved. The per- formance of poststroke rehabilitation patients making a hot drink was associated with the severity of neurological impairments. The errors occurred during making a hot drink process in the virtual environment and real life were compared for evaluating the neurological impairments accurately. The errors performances recorded by video were classified by catogories. Results The per- formance scores in the real world and the virtual environment were not strongly associated (p=0.30;P〈0.05). The performance scores in both environments were associated with age, ADL scores, Mini Mental State Examination scores, and tests of visuospatial function. The making hot drink performances in the real world were associated only with ann power and sequencing ability ,while which in the virtual environment were associated only with language function and practical degrees. Participants could make dif- ferent errors during completing this task in the real world and the virtual environment. Conclusion Although this virtual envi- ronment can be used in the poststroke rehabilitation, many difficulties exist and many challenges in rehabilitation are present. The virtual environment perhaps is not a more effective rehabilitation measure as assumed before. More virtual environment tasks need to be reseached and developed for evaluating their rehabilitation role to the stroke patients.
