摘  要:目的探讨利用SPECT行99mTcO4-异位胃粘膜显像,在小儿下消化道出血的筛查和诊断中的价值.方法对55例小儿下消化道出血的患者行99mTcO4-异位胃粘膜显像,由两位有经验的核医学医师判读结果.显像资料均参照临床最终结果进行评价。结果55例患者中显像阳性者11例,其中10例证实为麦克尔憩室,1例为小肠重复畸形。显像阴性44例,可追踪病例19例,经手术、消化内镜证实胃肠溃疡或肠息肉5例,麦克尔憩室1例,其余病例根据临床表现和治疗好转出院排除胃粘膜异位症。SPECT异位胃粘膜显像诊断胃粘膜异位症的灵敏度为100%,特异度为94.74%结论SPECT异位胃粘膜显像对小儿下消化道出血的筛查和诊断有较高的价值,且无创、简单易行、快速可靠,是临床筛查和诊断小儿下消化道出血病因的首选影像学方法。Objective Using SPECT go 99mTcO4 discussed-ectopic gastric mucous menrbrane imaging, under the gastrointestinal bleeding in children and the diagnostic value of screening. Methods 55 cases of children of the gastrointestinal bleeding patients go 99mTcO4 ectopic gastric mucous membrane imaging, by two experienced nuclear medicine physicians interpretation result, imaging material are refer to the evaluation of clinical eventually. Results 11 of 55 cases had positive result s. 10 of 11 with positivity result cases were found meckel's diverticulum. 1 case was double intestine.44 cases had negative result s. 19 of 44 with negativity result cases were proved by surgery and endoscopy or other methods , including 5 gastric ulcer or intestinal polyp ,1 meckel's diverticulum. Ectopic gastric mucosa was ruled out in others cases. The sensitivity and specificity of radionuctide 99mTcO4 imaging were found to be100 % and 94.74 %. Conclusions SPECT ectopic gastric mucous membrane imaging for children under the gastrointestinal bleeding screening and diagnostic have higher value,noninvasive , simple and easy , quickly and reliable. It should be the first choice of imaging in diagnosis of eetopie gastric mueosa.
