AFP联合三种肿瘤标志物检测对原发性肝癌诊断价值的研究

摘　　要：目的探讨甲胎蛋白（AFP）联合γ-谷氨酰转肽酶（GGT）、碱性磷酸酶（ALP）、癌抗原CA50检测对提高原发性肝癌（PHC）检出率的临床诊断价值。方法对77例原发性肝癌患者和61例肝硬化患者进行血清AFP，GGT，ALP和CA50定量检测，各组间进行对比分析。结果原发性肝癌组血清AFP，GGT，ALP和CA50水平与肝硬化组比较，差异均有统计学意义（P〈0．01），原发性肝癌组患者血清AFP，GGT，ALP和CA50单项检测阳性率分别为77．9％，63．6％，55．8％和66．2％，明显高于肝硬化组（P〈0．01）；AFP与三种肿瘤标志物联合检测阳性率为94．8％，显著高于TM单项检测阳性率（P〈0．01）。结论AFP联合GGT，ALP和CA50检测能显著提高原发性肝癌的检出率。Objective To investigate the test of alpha fetoprotein （AFP） combined with GGT, alkaline phosphatase （ALP） and Cancer antigen-50（CA-50） on the clinical diagnostic value of improve the test rate of primary hepatic carcinoma （PHC）. Methods 77 patients with PHC and 61 patients with hepatic cirrhosis （HC） were quantitatively measured the serum levels of AFP,GGT ALP and CA-50. AFP,CA-50 were measured with chemical luminous techniques, GGT and ALP were measured with the colorimetric analysis of rate. Analyzed the statistics of each group. Results According to the comparison between serum levels of AFP,GGT,ALP and CA-50 in the patients with PHC and those in the patients with HC, the difference had statistical significance （P〈0.01）, the rates of individual positive of serum levels of AFP, GGT, ALP and CA-50 in the patients with PHC were 77.9 %, 63.6 %, 55.8% and 66.2 % respectively, apparently higher than those in the patients with HC （P〈0.01） ;the testpositive rate of AFP combined with 3 kinds of tumor markers （TM） was 94.8%,apparently higher than the individual test positive rate of TM （P〈0. 01）. Conclusion The test of AFP combined with GGT,ALP and CA-50 can obviously improve the test rate of PHC.
