基于食物特异性IgG检测的溃疡性结肠炎患者个体化饮食健康管理方案研究

摘　　要：目的观察个体化饮食健康管理方案对慢性溃疡性结肠炎患者的临床疗效。方法对68例慢性复发型溃疡性结肠炎患者进行食物不耐受血清特异性IgG抗体检测,将59例阳性患者随机分为试验组30例和对照组29例,试验组予以个体化饮食健康管理方案+口服安慰剂,对照组予以个体化饮食健康管理方案+口服美沙拉嗪肠溶片,观察个体化饮食健康管理方案实施6个月后患者主要症状复常率、Southerland疾病活动指数评分及对慢性溃疡性结肠炎的疗效。结果 68例慢性复发型溃疡性结肠炎患者食物不耐受检测阳性率为86.76%,不耐受食物位于前3位的依次为蛋黄蛋白、虾、蟹,完成个体化饮食健康管理方案6个月者53例,患者主要症状复常率、Southerland疾病活动指数评分等均有明显改善;试验组完全缓解4例,有效17例,无效5例,对照组完全缓解6例,有效17例,无效4例,两组疗效比较,差异无统计学意义(u=-0.578,P>0.05)。结论基于食物不耐受血清特异性IgG抗体检测的个体化饮食健康管理方案在慢性复发型溃疡性结肠炎的防治中有重要意义,可为溃疡性结肠炎的临床治疗提供参考。Objective To observe the therapeutic efficacy of individualized health management plan for patients with Ulcerative Colitis（UC）.Methods Sixty-eight patients with chronic relapsing ulcerative colitis were given food intolerance specific IgG antibodies testing and 59 with positive results were divided randomly into groups test（n=30,receiving individualized food health management plan＋ oral placebo） and control（n=29,receiving individualized food health management plan ＋ oral mesalazine slow release tablets）.The normalization rate of main symptoms,score of southerland disease activity index（DAI） and therapeutic effects were analyzed after 6 months.Results The positive rate of food specific IgG antibody testing was 86.76%.The top 3 kinds of intolerant food were egg（white/yolk）,shrimp,crab.Fifty-three UC patients completed 6-month individualized health management plan,and their main symptom normalization rate,DAI scores improved significantly.In test group,complete remission was noted in 4,effectiveness in 17,ineffectiveness in 6,and in control group 6,17,3,respectively,the difference was not significant（u=-0.578,P〉0.05）.Conclusion Individualized health management plan can provide reference,by testing level of FI specific IgG antibody,for clinical diagnosis and treatment of UC.
