摘 要:目的探讨错配修复基因改变在大肠腺瘤中的表达,探讨其在大肠腺瘤发展至大肠癌过程中所起的作用。方法采用免疫组化的方法检测炎性息肉、大肠腺瘤和大肠癌组织中hMLH1和hMSH2蛋白表达和突变情况。结果大肠炎性息肉hMLH1蛋白阴性表达率为4.76%,hMSH2蛋白的阴性表达率为0;大肠腺瘤性息肉组织中,hMLH1蛋白阴性表达率为21.66%;hMSH2蛋白阴性表达率为8.92%;在散发性大肠癌组织中,hMLH1蛋白阴性表达率为27.27%,hMSH2蛋白阴性表达率为13.64%。结论本文的结果表明hMLH1和hMSH2蛋白阳性表达率从炎性息肉组织、腺瘤组织到腺癌组织逐渐降低,提示早在腺瘤阶段已经有MMR的基因的突变或功能异常,提示MMR基因的突变,尤其是hMLH1和hMSH2的突变可能是大肠癌发生的早期事件之一。Objective This study aimed to investigate the mismatch repair gene expression changes in colorectal adenoma in order to explore its role in the development of colorectal adenomas in the process of ascending colon cancer. Methods In this study,using the immunohistochcmical method to detect hMLH 1 and hMSH2 protein expression and mutation in inflammatory polyps, colorectal adenomas, and colorectal cancer. Results The hMLHI protein negative expression rate was 4. 76% in colon Inflammatory polyps, and hMSH2 protein negative expression rate was 0% ,hMLH1 protein negative expression rate in adenomatous polyps was 0. 2166;hMSH2 protein negative expression rate in adenomatous polyps was0. 0892; In sporadic colorectal carcinoma hMLH 1 protein negative expression was 0. 2"/27, hMSH2 protein negative expression was 0. 1364. Conclusions The results of this paper show that the hMLH1 and hMSH2 protein expression have decreased from inflammatory polyps ,colorectal adenoma to adenocarcinoma ,suggesting that the MMR gene mutation or dysfunction as early as the adenoma stage , suggesting that the mutation of MMR genes ,especially hMLH1 and hMSH2 mutation may be an early event in colorect.al cancer.
