Willis环变异与脑血管疾病关系的CTA研究

摘　　要：目的运用CT血管三维成像技术显示Willis环形态及类型,探讨Willis环变异与脑血管疾病之间的关系。方法收集行头颅CTA检查的111例患者,分析前、后交通动脉显示率、胚胎性大脑后动脉发生率、A1及P1变异率情况,将其分为正常组、动脉瘤组及脑缺血组,并根据Willis环的完整性,分为Ⅰ型、Ⅱ型、Ⅲ型和Ⅳ型4种类型,比较分析3组之间Willis环类型及变异情况。结果 111例病例中,Willis环完整共有17例,占15.3%,Wills环变异共有94例,占84.7%。正常组Willis变异率为85.1%,动脉瘤组变异率为86.1%,脑缺血组变异率为89.2%;动脉瘤组与正常组整体变异率比较,差异无统计学意义(P>0.05),但,A1变异率、PcoA显示率、P1显示率及FTP发生率两组间差异有统计学意义,A1变异率及PCoA显示率动脉瘤组高于正常组,P1变异率及FTP发生率正常组高于动脉瘤组;脑梗死组与正常组整体变异率比较,差异有统计学意义(P<0.05),A1变异率脑缺血组高于正常组,差异有统计学意义。结论运用CTA成像技术了解Willis环的变异情况,有助于探讨脑血管病发病机制,提高疾病诊断的准确性和治疗的安全性。Objective To observe the morphology and patterns of Willis circle in healthy Chinese by using CT imaging of cerebral blood vessels （CTA） , and to discuss the relation between the variation of Willis circle and cerebrovascular disease. Methods A total of Ⅲ patients with head and neck CTA were collected and divided into asymptomatic group, aneurysm group and cerebral ischemic group, and Willis circle was divided into four types： type Ⅰ , Ⅱ, Ⅲ, Ⅳ. The situation of three groups＇Willis circle were compared and analyzed. Re- sults In Ⅲ patient, Willis arterial circle normality appeared in 17cases, which accounted for 15.3% , while Willis arterial circle abnor- mality showed in 94 cases, which accounted for 84.7%. The variation rate was 85.1% in asymptomatic group, 86.1% in aneurysm group, and 89.2% in cerebral ischemic group. There was no statistically significant difference in variation rate between aneurysm group and asymp- tomatic group （P 〉 0.05） ; but in the variation rate of A1, the displaying rate of PCoA, the variation rate of P1 and the occurrence rate of FTP, two groups showed significantly statistical difference. The variation rate of A1 and the displaying rate of PCoA of aneurysm group were higher than those of asymptomatic group. The variation rate of P1 and the occurrence rate of FTP of asymptomatic group was higher than those of aneurysm group. There was statistically significant difference in the variation rate between cerebral ischemic group and asymptomatic group （ P 〈 0.05 ）. The variation rate of A1 in cerebral ischemic group was higher than that of asymptomatic group, and there was statistically sig- nificant difference. Conclusion Knowing the variation of Willis circle by using CTA can provide help to evaluate the mechanism of cerebro- vascular diseases and improve the veracity of disease diagnosis and the safety of treatment.
