急性缺血性脑卒中患者血清炎性细胞因子动态变化研究

摘　　要：目的探讨急性缺血性脑卒中患者血清肿瘤坏死因子(TNF-α),白细胞介素(IL-6,IL-8)等炎性细胞因子的动态变化。方法选取2010年1月-2012年1月某院收治的急性缺血性脑卒中患者128例为研究对象,用双抗体夹心酶联免疫法测定不同时期血清TNF-α、IL-6和IL-8的水平变化,另选取同时期该院体检中心87例健康体检者作为对照组。结果观察组发病当天TNF-α、IL-6和IL-8水平明显高于对照组,比较差异有统计学意义(P﹤0.05);随时间(d 3、d 7)延续,TNF-α、IL-6和IL-8水平逐渐下降,但仍高于对照组,比较差异有统计学意义(P﹤0.05);d 14TNF-α、IL-6和IL-8水平下降至正常水平,与对照组比较差异无统计学意义(P﹥0.05)。结论 IL-6、IL-8以及TNF-α增高介导的以白细胞浸润为主的急性炎性反应可能是急性缺血性脑卒中疾病进展的一个重要原因,它们之间互相诱生、相互作用,是急性缺血性脑卒中的高危因素。OBJECTIVE To study the dynamic change of inflammatory cells factors such as serum tumor necrosis factor （TNF- α） and intedeukin （IL-6, IL-8） of acute ischemie stroke patients. METHODS From January 2010 to January 2012, 128 cases of patients with acute ischemic stroke were selected as the research objects, and 87 cases of medical center physical ex- amination were selected as the research objects of control group. We determined the dynamic change of serum TNF-α, IL-6 and IL-8 in the different periods by double antibody sandwich enzyme league immune method. RESULTS The levels of TNF-α, IL-6 and IL-8 on the day of the observation group were obviously higher than those in the control group, the difference was a statistical significance （P〈 0.05） ; Along time continued （3^th day, 7^th day）, the levels of TNF-α, IL-6 and IL-8 gradually declined, but were still higher than those in the control group, the difference was a statistical significance （P〈 0.05） ; On the 14th day, the levels of TNF-α, IL-6 and IL-8 dropped to normal level, the difference was not statistically significant （P 〉 0.05）. CONCLUSION The leucocyte infiltration of acute inflammatory reactions that mediated by the increased levels of TNF- α, IL-6 and IL-8 can be a main reason in acute ischemic stroke disease progress, and are risk factors of acute ischemic stroke by interreaction among them.
